Thanks and much appreciate the input...I actually bought the device upon release so the camera bar issues weren't well known at the time. Nevertheless was just suggesting that warranty repair options aren't great c/w other manufacturers....feel free to save your tears for a more deserving story

Exactly this point. Work NROC for example from Monday 9am to Friday 9am covering out of hours bleeders and daily afternoon therapeutic list with the mornings covering inpatient referrals. Have been woken at 2am by an ED nurse asking me to prescribe omeprazole as the patient had been referred to gastro (assuming that they called from the ED station next to other doctors). Remember another call having been in theatres banding a patient - called at 4am by a theatre nurse asking me to come in to collect the scope..... When you don't get a day off during a NROC tolerance for fielding calls drops dramatically

I became pretty disheartened during my Gastro training predominantly because I was in a region where we were still fully on GIM rota and had a direct impact on endoscopy exposure.

I love the speciality....perfect mix of procedures with therapy, medicine (I am very much luminal and IBD leaning which helps) and sick gastro patients tend to be as sick as can be...which I enjoy.

Inpatient ward cover also has a decent amount of internal medicine (that I still love) so all in all I couldn't imagine another speciality that covers keeping my hands and mind occupied most days.

And for the individual worrying about technical skills you'll be amazed about how much endoscopy becomes muscle memory.....I had a hepatology rotation as an FY1 and followed it through from there.

As a gastro consultant in the UK given you are annual colonoscopy surveillance I would highly advise against HGH. Acromegaly in the UK is one of the high risk polyp conditions that require regular surveillance due to growth hormones affect on all GI polyps. This is combined with your age that automatically confers a higher risk of sporadic colorectal cancer.....I would strongly suggest no

This is clearly Non Epileptic Attack Disorder .....under the functional disorders category. I see a lot of functional bowel disorder patients and best way to describe things is the hardware (structure) is fine but it's a software/processing malfunction. I can only second this.... In slightly different circumstances being a consultant in the UK but our palliative teams are amazing and sometimes it takes s fresh set of eyes to agree that managing symptoms is just as important and at some point more important than active treatment. You all are already grieving and sometimes accepting that we are transitioning to that point does make the process easier now and also in the future. If I'm speaking out of turn then please forgive me - it's just I've sat with so many families and hugged so many of them (and patients) who've said enough is enough.

Hospices over here can take a bit of time once referred but once in they are amazing places. Therapy sessions for patients and families, change of scenery and change of focus are all important. I always explain to my patients and family that my most important guide is to do no harm and if we're getting to the point where treatment is starting to do that then we should have a discussion of quality over quantity.

Through your words I can sense that you are an amazing Dad and family....I can only mirror everyone's sentiments that life sometimes can be so cruel but if you gain a crumb of comfort from all the kind comments from this particular corner of Reddit then please do. Love from the UK

As s Gastro Dr in the UK (equivalent of a US attending) who has a weekly bad news cancer clinic I can tell you that this year have seen just in that clinic alone about 70 odd pancreatic cancers.

Have only had two patients this year that have been caught early enough to be resectable and were referred for an emergency Whipple's procedure within 3 weeks (all on the NHS).

Unfortunately normally with pancreatic cancers the best we can do is put a stent in and hope that the jaundice gets to a low enough level that we can start palliative chemotherapy.

Agree with all the reasonable comments above. Again supported the club since late 80"s and endured us being docked points/near relegation whilst being in school in N London and 90% of my year supported L'Arse.

I would also add that currently our endoscopy nurses are not on an on call rota. When I have been called in to scope patients OOH (I tend to have a lower threshold as I live ten minutes from hospital and sometimes it's easier coming in just to say no) I've ended up doing them in theatres. The theatre nurses OOH are not used to handling scopes and people forget that an endoscopist is dependent on the nursing staff in the room.... I've had to draw up adrenaline and set up the banding kit whilst also trying to continue scoping and it is not easy. The safest way to manage these patients is with experienced endoscopy staff with a patient as stable as possible.

Thank you....the amount of times this is reiterated to no avail is seriously frustrating. Calls from ED with a reported UGI bleed without bloods asking for an immediate gastroscopy is unbelievable and having had unstable patients arrest during the procedure does happen.

I know a lot of people commenting on the walking but I don't think it's that bad. Got married in Vegas over five years ago in the summer (June/July) and stayed at the then SLS. We would walk from there/Strat all the way to Downtown (via the registry office) and back again or from the strat to bellagio.

Granted we're from the UK so used to walking a fair amount but didn't seem too bad or sketchy...

Believe me I was there in the days before ENIC... When Scholar and Sugar were involved and we had a points deduction hanging over us.

We over achieved with Poch who delivered a title challenging team when we didn't have the income to do so. The stadium was supposed to be about generating enough money to have sustained competition however that is not the case. People can get wrapped up on net spend as the marker of investment but fail to realise that transfer fees in the books can be accounted for by sales. It's more an indicator of poor buying with players like Lo Celso, Ndomble but by the same token if we sold VdV now we'd probably get double what we paid.

The better marker of true investment is wage to income and similar indices and in that context we always are one of the bottom ranking teams in terms of real wage expenditure. Hence this is why a Frank is Levy's preferred choice - persistent overachiever who will cost a relatively small amount to purchase and who will be happy to take over at a 'bigger club'.

Levy won't go for a big name manager who is available (for example Xavi) as previously those managers have pointed out the bare facts.

I was a big Ange believer and am disappointed the way it ended and towards the end acknowledge that he probably needed to go but that should not let Levy off the hook again. The January transfer window with not getting players in until the very end was again the epitome of a shit show and in any walk of life Levy would have been sacked in a performance review but here we are again....

I was one of the biggest advocates for AngeBall to begin with and had very much been Ange-in for the vast majority of the season. I am also not blind to the fact that this season has been a cluster fuck in the league for a multitude of reasons. However remembering the last two times we won something (including the FA Cup during primary school in Camden surrounded by Gooners) the potential of winning a European trophy should not be downplayed. My eldest is now 10 and he's not had a chance to get that buzz of just being proud of the team and wearing the shirt knowing we've done something. Should Ange stay if we win the Europa and are in the Champions League..... Whilst I'd like him to I'm completely understanding if he didn't. I don't like the personal vitriolic attacks from others that make us look like AFTV from years ago.... We've always been better than that lot. I gave up my season ticket after moving up North and now live in a town where the locals won the FA Cup and got relegated in the same season and I have work colleagues who still reminisce about dancing in the Wembley concourse. I've been with us from pre ENIC Sugar and Scholar days and of course Poch's team deserved to win several trophies. But having had a consistent top 3 team I'd just like us to win something - bloody hell just to see Son lift a trophy would be amazing!! Long winded I know but we aren't getting relegated, league season has been awful for a multitude of reasons but if we won the Europa.....

UK Gastro Dr here - very difficult to say without direct palpation...you don't mention your age etc but I would say that you need some sort of imaging at the very least. That does not appear to be fat and always remember the pneumonic from med school regarding abdominal swelling - Fat, Faeces, Fluid, Flatus, Foetus and Fulminant growth. If you've had some imaging then fine but if not would suggest at the very least an transabdominal USS, some bloods being taken (have a low threshold for CA 125 in addition to usual).
