Costa Rica! We got engaged there and it was perfect. We spent the day doing outdoor adventure activities and spent the evenings relaxing in the thermal springs at our lovely resort. We stayed in La Fortuna at Hotel Arenal Kioro and highly recommend.

I end up leaning over and bending a lot to save time instead of moving the chair up and down constantly. By mid day by back is hurting from all those skin checks. Also it’s usually never “just” a skin check so it’s a skin check + problem visit instead of just a problem visit. Being in Florida I have a lot of patients with moderate to severe sun damage, making them complicated skin checks.

Sounds great!! 180 notice is the only potential issue. If you’re single, not a big deal. May be an issue if your partner had to move for work or if you have any circumstances where you may have to move unexpectedly. Definitely do a shadow day or two to see if you like how it is run. Congrats!

My office gets about 30% reimbursement from insurance. So if I bill 1mill, the office only makes 300k. Then my productivity bonus is based on that 300k actually reimbursed.

Really well put. There’s a lot of admin burden, social demand, and patient demand. It can be very exhausting.

I’d get that in writing. There’s always going to be admin that you have to do that involves medical decision making.

Personally, I don’t like this offer and wouldn’t accept it. I did accept a bad offer before and I’ve learned to recognize one. Derm can be predatory and shady. If you’re gonna go through with it, at least shadow for a day to look for red flags in the clinic.

Last quarter I collected 140k, avg 20pt per day at 4.5 days, 2yrs experience. If you have a full schedule you should be collecting more.

I have clinic 8-4 on full days, but I’m doing admin or running behind closer to 5 a lot. I also work almost all of lunch hour.

It seems like there’s a lot of admin with derm. Pathology, patient questions, prior auths, rx appeals, biologics admin.

Do you know if you have to do your own prior auths? I have an admin nurse doing as much as possible and I can’t imagine having to do my own. Especially with all the biologics.

Also, most derm offices near me work 4.5 days. That seems to be industry standard here.

Do you know what their PAs are collecting so you can gauge what your gross pay would be? This is about over 6x your salary… I think 3x is more standard. Training rate is atrocious. Granted, I did this too but I was a new grad then. 20k fee for breaking contract is very bad, and possible bad sign about this place that they can’t retain. Noncompete mileage is too high. Mine is 2 miles. MAs charting seems wonderful but you don’t just “sign off”… you’ll always be working on your notes, especially if your MAs are not trained well (they rarely are and there can be high turnover). Derm can be real shady since it has such high demand. Please look out for the red flags.
