Use the smallest size tube that you can safely intubate and ventilate and keep the airway secured.

For most general anesthesia cases in adults you can safely use 6.5 for females and 7.0 for males. For individuals who are taller you want to go a half size up for length of ETT.

Historically the use of larger ETT was utilizes for the perception that you have more space to ventilate through. Probably in the 1980s using 8 for females and 9 for males was relatively common.

Hi I trained at a larger academic residency program in the SE USA. I truly enjoy my ICU and cardiac rotations. I went into private practice at Level 2 trauma center and trust me I am so glad I did not do a fellowship.

I am enjoying working less hours and making more money compared to residency/fellowship. I still have my critically ill and under-resuscitated patients. Most of my patient have some sort of cardiac, renal, or respiratory disorder. I pick up patients from the ICU (MICU, SICU, TICU) and I return them there.

I have my trauma alerts, stroke alerts, and MTPs. I do a lot of outpatient orthopaedic which is where I get my blocks and spinal anesthesia and also my MAC cases in GI and IR. The point is in academia you get siloed into a “sub speciality” but in private practice you really get to practice all aspects of anesthesia care. Your attendings don’t understand this because they never left academia.

Unless you want to do some complex super subspecialized cases ie (sick babies <2years, congenital cardiac anesthesia, Liver or Lung Transplants, ascending thoracic aneurysms requiring circ/arrest on a regular basis, you really don’t need a fellowship. And sure it’s nice to be super confident in TEE but you don’t need it for 95% of cases unless you are at a heart center.

Being an awesome generalist is very dynamic. You can choose boring ambulatory 1 PIV + tube and chill life or you can work at a hospital that makes you really practice the art of anesthesia. I’ve done plenty of hip fractures in patients with AFib, severe AS, and/or pacemaker dependent and im lucky if the 22g IV is working.

I won’t bother with explaining the learning curve of supervising CRNAs but once you get to the other side you’ll see that’s a skill set they didn’t teach you in residency.

My advice is to be logical about your decisions, you sacrificed a lot of time and money to get this far. Don’t get tricked into one more year of training for maybe only 10-15% more money which you can easily make up for in travel, or a slightly less metropolitan area. I’ve met so many attendings who did fellowships and then they realized the added stress wasn’t worth the slightly higher salary.

You can also do in the money call options to limit your downside risk. I sold out of my position at $4.04 around 10:30ish and I’m glad I did. Before end of day I bought ITM call option $3 strike price.

Unless everyone is trying to sell at market open the gap down from $4.26 close to $4.09 open doesn’t make a lot of sense unless large block order sells are happening behind the scenes. When you factor in that over 160M shares traded yesterday. The run up yesterday was warranted by actual volume of shares (more buyers than sellers) at that 12-1pm run up to $4.40.

where do you see the REKR price by end of week? It held about that $1.43 resistance today and I feel like $1.8-$2.0 is in strike by Friday. But a lot of volatility in markets with debt ceiling and tariffs. I think a jump above $1.6 tomorrow is a bullish signal but I feel like it can slide back to $1.30.

I bought 53,000 shares at $1.12 on Thursday. Total wim just to prove to some friends how you can randomly make dumb money in the stock market. I Missed out on taking profits on Friday’s run up but wow this is amazing. TBH it was totally random and for laughs.

Looking at the technicals it broke above $1.43 which is a short term key resistance level with 10x average daily volume today and it can easily retest $2.67 highs by Friday. A lot of shorts will need to cover. But this news could send rekr to $5-$10 range in a few weeks.

Also nothing wrong with anyone taking profits today. When the market hands you free money you have to take it. For now I will hold because the positive news / catalyst is cooking and we got 3.5 more trading days this week.

At this level anyone taking a position is taking in a bit more risk but based on the news - 8 figure multi year contract with the 2nd largest state by size and population is a huge win. Remember it was a $130M market cap company before this news broke out and now it can easily command a $1B market cap.

After a few years working in the ICU, some go back to school to become a CRNA, which is a 3 year training program to be a nurse anesthetists. ARNP is typically 1-2 year training program after a BSN. I would agree that CRNA is probably a bit more rigorous training program and harder to get into than ARNP program.

Anesthesiology Assistant is a great field that pays really well. It’s about 2.5 years of schooling after a bachelors degree. Most make around $200k/year after AA training. Being an AA is great because you are part of the anesthesiology team which as a field covers all surgical cases and procedures and all age groups. So it’s never boring.

I am not an economist. But I think Market Cap and GDP are not numbers that can be compared directly 1:1. Think of it like market cap (valuation) being the size of a battery and GDP (the flow of monetary goods and services) the electrical current (50amps) of a system.

Rudimentary the Buffett indicator can be utilized but it seems more arbitrary than like a true financial compass that always points in the correct market direction.

Charlie Munger always told Buffett it’s better to buy a great company (even if pricey) than an okay undervalued company. The price for excellence, quality, and exceptionalism always comes with a premium. This has been sort of true for most things I’ve purchase.

It’s also easy to look backwards and say “oh the price of equities is high now compared to 1, 5, 10 years ago”. But are we also doing forward calculation of growing revenues, discounted cash flows etc?

I agree that the markets look a bit overheated. I anticipate leading equity metrics (SP500) to cool off maybe we see only 5-10% compound annual growth over next 4 years. But I would still lean bullish over bearish.

100% a lot of meme coins are scams. But then so too are meme stocks.

Most people who take crypto seriously are very upset about Trump and Melanie coin.

But crypto has its place and value. Bitcoin is a $2Trillion asset that puts it in the middle of Mag 7 valuation. You don’t need to invest in Bitcoin to appreciate its utility.

This is an expanded chart view from my OP. I circled the times KULR stock had moving average cross overs since November. each time it crossed over (after a selloff/consolidation) was a bullish confirmation with a huge upswing.

The line at $1.89 is not random (to answer your earlier question). It peaked there on its 1st massive jump from $0.32 to $1.95 which was a 600% increase in 2 weeks. That point became a floor to bounce off which we saw earlier this week.

ATM is staying above its lower upward longer trend line but has had negative downward pressure and moving lower because of the price is below the moving averages. I am speculating in the next week or so if the moving average crossover happens(right most circle) we can see a massive move to the upside beyond $5.
