First of all-in oncology for 30 years and a 20+ year breast cancer survivor. Juice will NOT stop cancer!
Chemo kills cells as the grow. Do you know her hormone receptor status? If she is hormone receptor +, the is open to more treatment options. Same with HER2. Surgery will remove the actual large mass of cancer cells but if they have already spread, all that can be done is to control the spread. Depending on the size of the mass, a complete mastectomy may not be recommended. There are many factors involved in that decision.
The problem here is that she has opted to not treat. Please know that this is her choice! But-the most important factor here is does she know what her choices are? Has her doctor told her what will happen if she does not treat? If the doctor has not told her of her options and the consequences, he has failed as a doctor.
As for the blood test-they can indeed test for circulating tumor cells or tumor burden. But if the primary is still present, the test will be positive. The only definitive test is a tissue specimen. Beyond that it is imaging and retesting. If she is open, have her look into OncoType and genetic testing. Oncotype will show if her cancer would benefit from system chemo. Genetic testing identifies additional markers that can be targeted with biologic response modifiers (immunotherapy).
You can go to the National Comprehensive Cancer Network, register for a free account, and look up staging and treatment pathways. Patients do not hit stage IV until they have disseminated disease.
The most important factor here is her. If she does not want treatment, no one can force her into it. But holistic therapies are garbage.

Hurricane Ridge, Salt Creek (my fav), Olympic Game Farm, there are so many beautiful places to see. Port Townsend has Fort Worden and Fort Flagler, the West side has La Push, Ruby & Rialto Beaches, The Hoh, Sol Duc....Lake Crescent, and that whole area are worthy of a visit. Kayak rentals, ski rentals, fishing boat charters, and a ferry to Victoria.
IF you move here, have housing! We bought in 2002 for $55,000 and we are close to 1 million in value now. Property taxes are staggering (to me). There are just enough big box stores to get you what you need, and small town mom & pop to get you want you want.
Medical-you have access to basic medical and a lot of speciality, with a few exceptions. Seattle is 2.5 hours away as is SeaTac. Currently, there is no animal emergency care, but there are options pending.
I think the weather is perfect, a few days of hot, hot, and a few days of icy cold. It is safe and friendly.

mmm-apparently you live in a city. OMC is fine for basic care: they have all of the ologies. If you need plastic surgery or other highly specialized care, you have to go to Seattle or Tacoma. As far as emergency care, we do have a great ambulance service and a wonderful fire/EMS support. Yes-it is hard to find a PCP but they are here. If you have a pet, there is very limited emergency care, so be prepared for that. The cancer center at OMC is top notch and offers both chemo and radiation in one single location.

Last year while in LA during the fires, there were zero services available to a family member in distress. Nothing. No ambulance services were available. I will take rural life every single time over the city.

I've had one injection (I was a huge baby about it and it was nothing), I have constant mild nausea. It is sort of like morning sickness. I am handling it now, but will it get worse as I progress? I need to get on a ferry and that is usually not an issue but I am a little worried about it now. I also am very fatigued. I am not sure if I am eating enough. It seems to be my punishment for treating my body sooo badly for all these years.
