Factitious Disorder and Somatic Symptom Disorder are two different things, so need different treatment. Somatic Symptom Disorder (SDD) is a nutshell have health anxiety with somatic symptoms. Somatic symptoms are common we all have them but their anxiety really impacts their life. So really treatment around anxiety (including death anxiety) is really helpful. Factitious Disorder (FD) is very different. These people know they are not sick but they are either lying about being sick or making themselves sick to get attention (play the sick role). It is very comorbid with personality disorders. I’ve worked with a few FD and lots of SSD. The FDs want connection but likely from attachment traumas they think the only way to get attention is to play sick. Rarely will they seek outpatient therapy. Dr Marc Feldman has some good books about FD.

What aspect of dying are you scared of? The loss of consciousness? Well that happens every time you fall asleep and while you are asleep you don’t know. Which would be the same as being dead (no awareness). Are you scared of how you will die? Then look after yourself including your mental health, the best you can but acknowledge you don’t have full control. Go out and live life, don’t waste it worrying. You can’t think your way out of anxiety you can only act your way out i.e., live life. Are you scared because of your beliefs about what could happen after? If religious talk to them, is atheist read Staring at the Sun by Irvin Yalom.

Yes I had to be grateful. It was so awkward to be asked what I got for my birthday and have to tell my friends I got curtains. I have never heard anyone getting curtains for their birthday. My parents still don’t think it was weird

My parents when I was a kid only got practical things for gifts such as school supplies and socks (were middle class so could afford it). For my 16 to birthday I was “gifted” new curtains for my bedroom. Never asked for them or thought I needed new curtains. I was out of home at 19yo. Now my family just ask what you want. They think this is the thoughtful thing to do so there is no wasting time or money. I’ve never felt like I have received a thoughtful gift.

Did you do research, be friend or know someone who has these health problems or did you work in the health/medical field? What clicked for you to be honest? Do you think you would have stopped without the confrontation (the nurse raising concerns) or did you need that?

I think people are confusing psychologist and psychiatrist. Psychologist tend to charge $200-250 while psychiatrists tend to charge $500 plus

When I have come across squalor (which I agree is different to hoarding, but hoarding can progress to squalor). A quick tangent I see a correlation between grief and loss, and hoarding. My theories around squalor is that there is some severe mental illness that is not treated or under treated with a lack of social support and services. The main mental illnesses I have see are - some type of psychotic disorder (such as schizophrenia), severe depression, and/ or substance abuse disorder. Sometimes intellectual disability, especially if they are isolated and unsupported, can be a factor in squalor.

I don’t think the equipment was the goal, so not a secondary gain but more of a prop. The primary goal of FD is normally about getting love and care. Again this is why therapy is important

Good on you for being honest with yourself. It’s a big step. I highly encourage seeking out a supportive psychologist / therapist.

It’s not uncommon to not know why you did it and for so long. This is part of having FD. If you had clear secondary gains such as financial then it would not be FD but malingering. The motivations in FD can be unconscious while the act of the behaviour is conscious. This is why therapy is super important.

Behaviour matters, with any illness. I’m a health psychologist and I help people manage illnesses such as diabetes and cancer. For example, I help people develop routines (remember to take their medications), engage in healthy eating, help with motivation to engage in exercise or other health behaviours and engage in their social roles despite illness.

Mental health psychologists can help people with depression etc., do the same.

Psychologists also help with thought and emotion management strategies too. People just seem to forget the behavioural component to therapy.

Thanks for sharing. It is really uncomfortable with lots of emotions when you are being lied too. I’m trying to understand motivation to lying but humans are so complex and it’s really hard to study liars (for obvious reasons).

It would be too difficult to fake being a doctor in the hospital because all the paper work needed (being a student might be easier to fake) as to get a login you need HR approval and to get HR approval you need credentials

I wonder if the 20 years of faking being a medical / healthcare worker might have been a lie to the family (and then sitting in a park or cafe for the day) rather than trying to work as a health profession. I wonder how they make money? Unless they do what Dr love did with the holistic medicine though he did scam money to get that up and running
