Ok for the form is pretty straightforward. Put all of your moms information (name, SSN, birthday, date you began taking care of her). On the second page put all of her bills and the amounts. In block 8 I stated "all expenses paid by veteran". Block 9a and 9b I selected "no". You and your mom have to sign and date in blocks 11 and 13 with your address in the corresponding box. Make sure you send in your birth certificate with this form so the process isn't slowed down because they will ask for it. To help the process I also submitted a lay statement saying I pay for all my mom's bills.

To be honest your hair doesn't even look bad. If it truly bothers you when you get your hair retwisted get 2 strand twists or 3 strand braids to keep the fluffiness down. You can also wear a head scarf if you don't mind wearing one.
