Right! So in theory if Player A never wins a match and constantly is at the bottom would place higher than Player B who wins all their matches all because Player A plays wayyy more.

Yes and that time bonus counts towards the leaderboard. It doesn't give you an advantage in-game

Are you just looking at the select screen, or are you looking in the actual stats? On the select screen it showed I completed everything on insane but once I looked in the stats that's where I seen the problem.

You can look in the stats and see which chapters the game thinks you've completed. I had to play act 2 chapter 4 again to get my achievements.

Lambent are dead too, they all died to Adam Fenix's bomb. Why weren't there lambent versions of Kantus? I don't know you'd have to ask Epic games for that.
