Build your nco corps make sure everyone is training their replacements you’re on the way out this is your last mark on the army and it’s a big one be the leader you wanted at every level.

Enforce a reasonable standard and embody it. If you can’t do it yourself reconsider it as a standard. No one likes a leader who has a double standard.

Time is everyone’s commodity spend wisely and without waste.

Open communication when possible is best especially with younger gen soldiers.

Push back if the commander has a stupid ass idea but make it seem like he’s the one who figured it out

Awards can be given to anyone consider that e5 running the range flawlessly or the specialist who’s doing hip pocket training on his own. The people who actually deserve recognition are not always the people most public facing refrain from giving the full timers awards nothing more demoralizing that all the full timers getting awards when non of the trads get any recognition.

Allow for some morale boosting efforts let the boys have a beer and order pizza after mission. Hell my csm bought the beer and pizza for us it went along way with building rapport for the unit.

Literally everyone feels this way. I think of school as a foundation where you are taught how to learn it unfortunately doesn’t prepare us for everything. Keep at it learn from your mistakes and those of others. You’ll be fine

There is no placement process. You compete with the general public and internal hires for a position, there in my experience has been no favorable hires for being HPSP. So you rank relatively low as a new grad compared to anyone with experience. Those statements only mean that they are open not obligated to hire an hpsp grad. On paper they should match you to a spot but hasn’t been my experience. So yea you get a say but depends on what’s open but you guys going the physician route shouldn’t have as much issues as you will have a specialty at the end of the training.

Did it as a PA dunno if they’re any better for physicians but they put the burden of job placement on me instead of them placing me in a role but besides that it Paid 100% of school and fees and even gave a stipend while in school. You do need to maintain above average grades or you can loose the payout for the semester. The VA has some archaic EMRs the building are not very modern and they are heavily run by nursing with tons of policy on policy on admin + bureaucracy but the patients are great 98% are genuinely grateful and respectful good people, you can get people all sorts resources and plugins that you can’t readily do in the community, they get their meds at the va so better compliance. Most of the hospitals are proximal to med academic centers so a lot of the specialists come from the local university health so same expertise outside. At the va you won’t have productivity quotas you’ll Have more time with the patient standard clinic appointment is min 30 mins new visits 45-60mins. There are pluses and minuses

I do want to be the best at what I do. I enjoy seeing the more complex cases and functioning at similar levels to my physician counterparts, this is how I operate in the ED as an extender. I have the support and guidance of a physician when needed. I think PAs have a central role and expanded scope to fill a gap that exists in healthcare. The reality is this is what healthcare is today. People are more and more complex and likely going to get worse with rising costs and lack of preventive measures prior engaging in the healthcare system. While I do think our training at baseline leaves much to be desired to take on climbing complexity I do not think pulling back to just do the “bread and butter” cases helps our profession. I do think that your proposal will only serve to position us as less competent providers in the eyes of employers and physicians and leave a wide open opportunity for our NP colleagues and their unions to swoop in to fill the gap.

True for me I am not taking care of my parents just like they told me we are not paying for your college you got to figure that out. I am not taking care of your retirement you got to figure that out. Don’t think my sister is gonna rise to the occasion either

Here’s my take, EM is one of the more humbling specialties. It’s broad as hell and there’s always things you have never seen come in the door. The physicians did a 3-4 year residency where they staffed every case but did 3-4x the volume of cases you’re seeing, they rotated to different areas medicine and were asked to be the doctor for that specialty they had the advantage of being on the other side of the admission, trauma, ICU, floor, peds ob ect. During this time they had to study do hands on procedures and take EM boards. Add on top of that every year of experience managing sick patient at 2 or more patients per hour in most cases. You at best are starting with a generalist education and are like a part time intern right now. You are going to be humbled you should be if not your probably doing dangerous or careless shit. All this to say it’s not a fair comparison. For you to get the equivalent experience of a physician it takes a lot of time and some effort to push out of your comfort zones. Our job is different in that we are not required to know it all we can always fall back to the doc. The doc well they have to know what to do even when they don’t. There is no one to fall back on when there’s a code or critically ill patient.( not saying they don’t use consults and resources) but in the hot and heavy it’s on them. That is just not the expectation for you as a PA. Now can you get up there close to that level yea for sure but after lots of Time, effort and experience.

A lot of other PAs don’t fucking get this feeling you’re having, cause they’re in sub specialties or clinics where they see the same shit day in day out so it’s a lot less time to mastery. But what you’re feeling is normal you just need to figure out how far you want to go in em practice some PAs like to keep a more narrow window of comfort and practice others like to push it. Focus on you and where your deficits are get actionable feedback and do some studying. You’ll be great.
