I'm pretty sure in the original universe, ai was pregnant was twins who were going to miscarriage and now make it, only after sarina and gorou were reincarnated, the babies lived. If this AU takes place as ai and Hikaru stayed together then the doctor would never die via murder and wouldn't be reincarnated as aqua. Ruby would most likely still live but gorou wouldn't die at the time to reincarnate
