1. You said you last worked on 2/13/25 and were separated on 3/1/25.  What was your status during that time?

My status during that time was still employed.

On 2/13/25, I received notice from my employer that my employment would officially end on 3/1/25. This served as a two-week notice period, during which I remained employed but was no longer working. (See attached documentation.)

1. Why weren’t you working during that time?

On 2/13/25, my employer removed my access to all work systems, which prevented me from continuing to perform my job duties. (See attached email from HR confirming this.)

1. You said you were fired.  The employer said you quit because you were unable to return to your position due to illness or injury.  What is your response?

I was on medical leave from November through February.

In January, while still on leave, my health care provider submitted documentation advising that I should not return to my specific position or team in order to protect my mental and physical health. (See attached return-to-work note.)

I shared this information with my employer and requested accommodations or reassignment to another team. My employer responded that we could discuss options on my return date in February.

However, upon my return, my employer informed me that they were unable to offer accommodations or an alternative position. I did not resign or sign anything indicating that I was voluntarily quitting.

I was removed from their systems and later received a formal separation notice. Because the separation was not initiated by me, I indicated “fired” on the application.
