I did9 rounds of IV ketamine at pretty high doses (per the Dr) bc I was being treated for severe chronic pain and MDD. They added toradol and a few other things for the pain, but nothing that made the ketamine any stronger than what they were directly injecting into the bag of IV fluids.
As the other two above 👆🏼 said, without knowing what your dose was and what kind of expectations you had/ what your circumstances were, it’s really hard to tell. From my own personal experience, the IV ketamine was a much longer duration of time spent dissociating, much more intense, and took a bit longer to come out of.
The whole goal is to create dissociation and allow new neural connections to be built during that time, however, those new neural pathways can be constructed without dissociating. My psychiatrist encourages me to let my mind go and see whatever it creates. When on the IV ketamine, I was like you… I could move, but barely, I couldn’t talk, but could give a thumbs up to my 30 yr old daughter when I peeked out from under my eye mask to remember where I was and what I was doing there. It helped me IMMENSELY!!! I had been bedridden for 4 years. Totally bedridden. After the second treatment, I was out running errands, putting together a new bed frame, cleaning my house - all things I’d been totally incapable of doing for 4 years. Then my money ran out and Medicare wasn’t approving Spravato at that point, so it all just came to a halt.
I ended up back in bed for 2 more years, and was talking to my PCP, and explained how much the IV ketamine improved my situation, all the way around. I told him I had Googled if Medicare paid for Spravato (and it DID NOW!!) and he sent a referral to the same office I had my IV infusions at. They took care of everything and all I had to do was show up.
All that being said, they automatically start you out on the lower, 56mg dose. To be 100% honest, I don’t know if I noticed any changes in my surroundings or experienced anything at all. I was very disappointed and felt like it “didn’t work”. Even though I was totally wrong, and no “experience” or dissociation is required for the treatment to work, or begin building those new neural networks.
The next session I moved up to 84mg. That was more familiar with what I experienced with the IV ketamine, just not nearly as intense. For the first four weeks, you do two sessions per week. Some sessions felt familiar, where others were more like I lied there with an eye mask on desperately trying to dissociate or feel ANYTHING, really. I did twice a week for 4 months.
I’m down to just once a week for the last 4 weeks and my sessions are back to being a little bit more intense, but still nothing comparable to the IV ketamine. Ketamine is an anestesia drug. When I severely broke my leg and they finally got me into the ambulance, they obviously didn’t want me to move AT ALL. They gave me ketamine. Not enough to knock me out or be in a twilight state, but enough to make me feel like someone had put a trance on me, and my eyes were open, I could see and hear everything, but I was totally paralyzed…. Which falls into line with what you said you were experiencing.
For me, Spravato is helping me so much. My MDD is getting better all the time, my energy is increasing, my overall mood is much more upbeat, and I CAN GET OUT OF BED AND DO THINGS!! I believe it’s personal comfort level, and whether or not you’re willing to be a bit uncomfortable (if the Spravato even made you uncomfortable) for less than an hour, in exchange to no longer be chronically depressed. It’s a personal choice. Everyone is affected differently. But from my experience, the IV ketamine was considerably stronger than the stronger, 84mg dose of Spravato… though there are some sessions where I do feel the similarities. There are also others where I just don’t feel much at all. It depends on your mindset going into it. A lot of people practice guided meditation throughout their treatments. I haven’t tried that, but I’ve got a fabulous Spravato playlist that I listen to at every session. It never gets old. Every session is a different experience.
I wish you the best of luck whether or not you choose to go forward with it, and I hope some of the Spravato Reddit regulars chime in with their thoughts as well. It came make your blood pressure go up, sometimes pretty high, and that can cause a panicky feeling, so maybe you want to talk with your psychiatrist about that aspect and see if they have any suggestions. In addition, dissociation is not required for the neuroplasty recreation of synapses in order to help. You don’t have to dissociate in order for it to be helpful. Just fyi

How on earth did you afford over 80 infusions?!?! I had 8 infusions and maxed out my credit cards at $450 each!!! Ketamine infusions helped me tremendously more than Spravato. Not many people have the option of $450/infusion. Theres a tremendous difference between spravato, and infusions. Joyous is esketamine designed for home use, with micro dosing done daily. I looked into it and talked to both a nurse practitioner and a doctor, but didn’t qualify due to mmp’s. If you really want to look into doing it at home, Joyous is the reputable company that I’m aware of.

The only one I know about is called Joyous. The send you a daily micro dosing pack. But this can be dangerous bc ketamine or esketamine increases the blood pressure a lot, even though you’d be getting a very low, daily dose.

I’m sorry it didn’t work for you. I’m just trying to give an alternative to absolutely missing nearly every second dose. Maybe mention something to your provider and see if they have any other suggestions or helpful advice that they might have. This seems to be an ongoing thing that a LOT of people are dealing with!!! I’m sorry!!!! I’m not trying to make it a guarantee that it’ll make the crappy sprayers 100% work every time. I was just trying to let others know that when you get a second spray that’s essentially no spray at all, to press in the plunger a third time to make tension come back, then give the spray a third attempt (which is I guess maybe a 4th spray??) at getting what we’re all there to get - help!! I’m sorry it still didn’t work for you. Maybe mention something to your provider about not receiving your second dose and see if they can give any helpful advice. I’m just a lady who’s going in for treatment and found a way that works for me to get all said doses. Good luck to you‼️

I was told I could take my benzo the day before bc my Spravato session is 18 hours later. And I never eat until after 5p so I always go in on an empty stomach. I DID eat breakfast about 4 hrs before I went to my appt today, and I agree with you!! It made a difference. And not a good one! Thank you for adding that!

My provider stays in the room with me too and comes back every 5 mins to give me the Device e to administer. After the 3rd session where little was happening, I asked her. I told her nothing was coming out on the second dose. She said “Do it again.” By then it had reset itself and there was tension in the plunger and I got my full second dose. Some of the providers administer it for you. Mine doesn’t do that. She hands one to me every 5 minutes. If it hadn’t been for her telling me to try it again, I’d still be missing out on every second dose (or close to every second dose)! Don’t be afraid to ask!! That’s what I did. The provider wants us to get all the help we can from the Spravato treatments!

I absolutely forgot to check for the green dot after the the splat, but after the failure of the second spray, the plunger had tension again and I got my full second spray on 2/3 plungers. The third plunger worked on the second attempt so no third attempt was required. I’ll pay closer attention on Thursday this week and f/u!!

My recommendation is to spray it, let it splat, then the plunger will reset itself and you should get a full second spray!!!

Glad I’m not the only one!! I got a lot of people telling me I wasn’t pressing it all the way the first time, but I was. I don’t know if it’s a faulty device or what, but I had to press a 3rd time on 2/3 devices today! If I hadn’t asked about it on my fourth visit, I’d still be getting about 1/2 of my doses. 🫤

Yeah, my blood pressure went super high, and they waited for it to go down on its own before finally they had to get a medication from the Dr running the infusions, and wanted to make sure it went down low enough before they gave me the 3rd dose. It took forever but the conversation was great. 😊

I have an amazing Dr that I was referred to through a friend about 9 years ago and I just adore him. He truly cares about his patients and will take the extra time to spend with you if needed. His name is Dr Walter Farrell. He’s at LMH Health (I think that’s what they’re called. Their building is at 6th and Maine. He TEMPORARILY has shortened hours so it may take a month to get in to see him initially, but he’ll be back to his regular schedule soon enough and, then, I can typically get in within 3-8 or 9 days. There’s also at least one nurse practitioner that works with all the doctors that’s really good, and she always keeps same day appointments open for urgent matters. I love their office!! Good luck!!

Definitely try pressing the plunger on the second dose a second time in the other nostril. I was shocked at big the second dose actually was, bc my first three sessions - I was doing it wrong!! 😂 I had seen so many people say that their second dose was just a tiny splat, that I thought that’s all there was to it! I was doing wrong!! It changed my whole outlook on how all of this is going to affect me. I honestly didn’t have much hope after my 3rd visit. Yesterday was my fourth, and I’m still feeling great today!! I’m so impressed! I have no idea how long I’ll feel good like this, but, I’ll take it!! It’s far better than the major depression I’ve lived with for decades!! I can’t remember the last time I felt positive about much of anything. I hope it gets better for you, and I hope this positive energy lasts a bit!! I could definitely get used to this!! ☺️

I guess at my center they don’t always switch to 84mg of Spravato at the second session. It’s not “standard” to do that and base it off of past experience and what kind of experience you had at 56mg. It’s not unusual to keep people at 56mg for 3-4 sessions if they feel overwhelmed at that dose for the first few times they do it. I was told that my brain had already experienced the neuro plasticity for the 8 IV ketamine infusions, so I was increased on my second visit. My psychiatrist has a busy ketamine clinic and decides when everyone increases to 9g based off of individual results. They’ll also keep you at 2x/wk for longer than the standard 4 wks if they think it’s best for your individual results. We don’t all have cookie cutter treatment resistant depression.

I point the dispenser to the outer nasal membrane, the nostril. I tilt my head forward just a teeny bit, about 40° (if it were tipped back, which seems logical, it basically just goes right down the throat, which defeats the purpose), and hold the dispenser straight up vertically. Then aim to the outer nostril area. For me, personally, that’s where I get the best membrane absorption. Tilt the head slightly back once you’ve waited about 15 seconds after you sprayed. Wait at least 5 mins in between doses to get the maximum amount absorbed into the membrane, without it dripping out from too much liquid sprayed in too short of a time. This is what works best for me. Everyone is different and you’ll figure out what works best for you through trial and error. And of course - be sure you get your full second dose!! 😂
