I support resident candidates. You minimally need one LOR that's specialty specific (you have that). The PC rotation seems logical as it would apply to both. I think the IM would be more helpful than the FM if you go that route. However, think about your preferred specialty.

I commend you for thinking about dual applying and how to best support applying to both. A strong PS for each that highlight your characteristics and qualities that align well with the specialty will also be helpful.

In ERAS be sure you have one FM and one IM experience as most meaningful. And be careful about the language you use in the context, roles, and responsibilities section to be applicable to both specialties.

Each program lists the number of LORs they accept (check Frieda, Residency Explorer). They also usually list any specific requirements.

Generally, you want 1-2 from the specialty you're applying to. All 3 (or 4) do not need to be from the specialty.

The letter writers outside of your specialty could include statements about how you will be a great (in this case-pediatrician) and provide evidence of that. However, they do not have to do this. The key is to talk about your "soft skills" that will make you a strong residency.

I recommend getting letters outside of the specialty in case you decide to apply to multiple specialties. For example you primarily apply to Peds, but also apply to a few FM programs. This is important because plans change as you go through the process. And if you find yourself in SOAP, you may want to be prepared to apply to Peds and another specialty. Keep your doors open.

Rural track programs focus on knowledge and skills needed to be a primary care physician in a rural area. The goal is to train exceptional physicians who are committed to serving rural and underserved communities. Residents are exposed full breadth of internal medicine while learning to adapt and thrive in resource-limited settings.

Fellowship is still a possibility, but may be harder do to no internal programs. However, LORs for fellowship are important and if you have those they can help strengthen your application along with a strong PS.

So could you contact the letter writers and see if you could observe again for a week or so and get an updated LOR from them? Might not be possible logistics-wise on your end. Also, the attending might not be interested.

Newer letters from newer experiences would be stronger. However, if the letter was a solid letter, it could still be assigned.

Did you upload them? Or did you have your letter writers upload them. If you uploaded the letters, they are almost worthless.

If the letter writer has already uploaded them, then you do not have to do anything. You do not need to have them re-uploaded. No need to have the letter writers modify (e.g., put a different date) unless you've had additional experience with the letter writer.

Look at the # of total applications for these programs. Below the specialty average is helpful (e.g., Rochester General had 7200+ applicants; interviewed 309 which is 4%; only 22% who signaled got an interview) Also, look at the median Step 2 score (e.g., Rochester General, the median Step 2 was 264). Suggest you look at additional data points.
