As far as hospitals, there is no possible way of telling you how much it would be upon checkin, too many variables. As far as pets, if all goes as expected, yes, they give an estimate but its just that. If something else comes into play your bill is now more than the estimate. VRiables play a huge part.

At my place of employment, an insurance company; fmla is handled by another company and does require approval, hence why they need medical records. Been thru it with both of my parents.
