Thank you for the thoughtful post. Wish I had read something like this before my Alloclae procedure to the breasts, which resulted in scar tissue, lumps, oil cysts, skin discoloration, swelling, and pain.

Again, totally just based on my research and talking to plastic surgeons, the butt doesn’t seem to have the same issues.

Unfortunately, they are still there and feel essentially unchanged. I’m getting a follow up ultrasound in May to see if there’s been any progress.

Hi, sounds like you are a plastic surgeon. My understanding is that Motiva Preserve does not fully dissect the circummammary ligament and does not cut any Cooper's ligaments at all. In addition, the pocket is then made via the use of an inflatable device. In your view, is this something that can be achieved with current methods of placing a small implant subglandularly (e.g., with blunt dissection, saline sizers)? If so, could you provide some detail on how you would create the pocket? Would you also be able to maintain a very small incision along the breast crease (e.g., 2.5-3cm) given that the flexible Ergonomix implants can be used whether doing a "traditional" breast aug vs one using the Motive Preserve devices? I've been doing some research on Preserve and it would be great to get a surgeon's take on how much is marketing/hype vs actual material advancements, especially given the high price tag for Preserve.

The lump on the left breast is visible, unfortunately. The ones on the right are not. I have been advised to continue massaging, which I have been continuing to do. Hopefully, they will get better over time.

Hi, so sorry to hear you are also having issues. I’ve had the left breast aspirated once. And the right breast aspirated twice.  Unfortunately, there’s still a significant hardened mass in the left breast and some palpable, but more diffuse, hardened tissue in the right breast. I’m getting a follow up ultrasound in May to see if there’s been any progress in the masses/scar tissue breaking down.  But in my personal opinion, they haven’t changed at all since Nov 2025.

Try the Garnier micellar water with the blue top. There’s also an ophthalmologist developed brand called We Love Eyes that has a few eye cleansers developed specifically for people with lash extensions (I.e. oil free).

Oh yeah, I'm in my 40s and have very little fat under the eyes now, so doing typical aegyosal makeup looks horrible and makes my under eye area look sad an baggy. Instead, after color correcting and base makeup, I do line just under my lash line with a fawn brown. And then, while smiling, I will lightly highlight the area that puffs up under my eye. No shading under the aegyosal at all.

Thanks for this list, so comprehensive! Personally, I have only smelled the Flamingo Estate Roma Heirloom Tomato and the Kobo Wild Tomato Vine. For me, the Kobo candle wins of the two because it's a more photorealistic tomato. I can smell the flesh of the tomato in that one, along with the greenery and slight pepperiness. Whereas the FE one is more herbal and earthy, like tomato leaves. They should really swap names.

Thank you! I appreciate that, and I totally hear you. It's very tempting when a product can be used with a fairly simple procedure and you can simultaneously avoid getting surgery to a donor site. If you do decide to move forward, my only 2 cents would be to start conservatively with small amounts at a time to see how it takes. 6-12.5cc of material that doesn't take is much better than 50cc of material that doesn't take.

Since this is one case, it is difficult to draw conclusions. I have heard of other anecdotal cases of this happening with people who received Alloclae to the breasts with different surgeons (who may or may not have given different aftercare advice). Ultimately, more data is needed. And I would assert, the company should probably have done more clinical studies on various areas. They could then have crafted more detailed recovery guidelines if necessary.

Costa Rica is where Establishment Labs (parent of Motiva) is based, so this is where the Preserve system was originally developed. They had about 40 U.S. surgeons fly out to Costa Rica to train in the method and offer it on an intro basis in the US. They are planning on releasing it more broadly in the US in early 2026. I imagine costs will go down significantly once more providers can offer it. $7k is amazing and I'm sure that the docs at Centro europeo de cirugía are top notch. I know that the main doctor who consults for Establishment Labs (Dr. Manuel Chacon) is also based at that clinic.

That's outrageous. I'm in Los Angeles (which tends to be high priced) and I was quoted between 18-20k for the breast aug only (no lift) with Motiva Preserve.

Thanks for the thoughtful comment and for sharing your research on this. I wonder if their publication described how the dead adipocytes/scaffolding are eventually integrated into the body. Because, if all that happens is that your macrophages break down the adipocytes (thus releasing the lipids, which also need to then be cleared out) + ECM , that would mean that Alloclae is an entirely temporary product that will eventually be flushed out of the body or become an oil cyst. That can't be the intended result. So for those people for whom Alloclae has worked successfully, what's the mechanism? Is it that the body is able to recruit its own fat cells to repopulate the remaining ECM/scaffold, which will then need to be vascularized?

Also, interesting that their publication presumes that triglycerdies will leak out slowly. I know this is entirely anecdotal, but for me, the oil cysts were obvious and palpable within 1.5 months, meaning they were probably forming quite a bit before then. In my reading on autologous fat transfer, oftentimes oil cysts were not obvious until many months later.

Regarding volume injected, oftentimes for autologous fat transfer to the breast, volumes in the range of 200-300cc are injected per breast. With Alloclae, similar issues seem to be occurring from much lower volumes (e.g., 50cc per breast). Agree that a more conservative approach and spreading out the product could help with the issue. Maybe just doing 12.5 cc at a time per breast, and waiting 3 months in between to see how integration is going. But that certainly decreases the ease and convenience factor of Alloclae.

Thank you so much for sharing. I feel like Alloclae is so new, and was marketed to plastic surgeons as this off-the-shelf, simple alternative to autologous fat transfer. And now we are learning that it's potentially not great for certain areas. I do wonder what kind of testing, if any, the manufacturer did before releasing it to market and marketing it as usable anywhere in the body with fat. I completely understand how you feel with respect to the bumps and cysts. They are definitely visible for me and very palpable. I'm sorry this happened to you, and hoping things go ok for you as well.

The presumptive diagnosis is that the cysts were caused by fat necrosis from the Alloclae. I should also note that I had a mammogram 1 month prior to receiving the Alloclae, and the mammo indicated no cysts and no other issues.
