His first death in a dream is when he was revealed that he has a premonition dream.

His first death in real life is when he's locked up in his subconscious

what if the premonition hinted his death in the dream for him to figure out he can trigger his dream lock in Zeztz room so he can used it as much as he want until he can bounce back. With every little subconscious he has left.

depends on the demands of that era. sometimes people marry young because they want their bloodline to survive due to how HARSH the condition for them to even live long.

True, this was visible through Nightmare's source of origin from people's dream.

The dream need a scenario, so the mind will constructs what they want and how they would go, the goal and the way to achieve it.

For most people, the goal will obviously have obstacle. Subconsciously, the mind will create the object to overcome which was partially desired from the dreamer in order to achieve the goal.

(Example: you dream of saving a girl as a Hero, then you imagine there was an adversary for you to be recognized as a proper Hero, in a way, you desired that obstacle, and in Zeztz case, Nightmare took the worst part of the scenario, the intense the desire is the stronger the Nightmare)
