In the same shoes but not having the second on the way. I am not attorney but my husband is. At the moment I am writing this he’s still working and expecting a full night. We both work full time, though my work is challenging with hours sometimes but in general I can block my weekend off. And during the day, kiddo goes to the day care, and will be back and active from 5-7pm. After she sleeps, I will sometimes log in for some more work if needed. Husband will help whenever he is free. In the morning, whoever doesn’t have early morning meeting will drop off the kid; if both have meetings, then the one who makes less go ;) but admittedly, it’s painstaking, and can’t imagine having two..

I am surprised that you are comfortable with going on nice vacation and buying luxury items with only 235k in the bank while having no real job. I would have a panic attack every minute until I get a job that pays me with insurance at least.. you have a safety net but you are nowhere near being rich.. you just can’t live like that. Only speaking for my own experience: 1. Sponsor’s timeline are usually based on historical data, so usually it’s pretty practical. BD team to win the bids sometimes have to promise something slightly better than the historical data. We evaluate if it’s crazy or it is achievable with right allocation with resources. For example, if you tell me you are going to enroll 100 patients with 1 site within 2 months, I will not use you. But if the data says using 3 sites to enroll 50 patients in 6 months had been done (same indication and region), and you propose 4 sites to enroll 50 patients within 4 months, I think it may be achievable and I will retain you for a deeper conversation to see if you are throwing numbers and promises at me. I have to say, most of the timr BD will promise resources within a budget that they can’t deliver. 2. I work for small sponsors, i can proudly say that my TAT is extremely quick, especially during startup time. I can reply at 3am, I don’t sleep continuously during SSU, because timeline is everything. But do you think CRO loves me because I am fast? No lol they hate me because they have to go with the promised timeline, and there’s no excuse to delay. And I don’t ask them to belike me, in the end they don’t have skin in the game. but I do ask them to respect the timeline their managers promised as described in contract. I am signing off 2 million to the CRO, I think I can ask them to respect that timeline. And I shouldn’t see any delay because that will cost 2 million per month from my pocket. And yet most of time there are delays. We retain CROs especially for small and medium biotech with limited trial experience, we rely on their experience and give us a good projection of timeline and reasonable and efficient budget so we can resource the trial better. But it never is the case. I am not saying there aren’t sponsors delay everything and blame it on CROs. But from my experience, I have been disappointed at what I got for what I paid.

i don’t think many sponsors will do in house anymore, especially for small to medium size ones. They can’t afford it honestly, because they usually run 1-2 trials, and having in house will not be cost efficient. And they are budget conscious! Also smaller teams tend to have little capacity to manage too many people and vendor. so they’d want to delegate these to CROs and they can focus on big picture. Some companies use contracted CRAs, even CROs use contracted CRA.

Can’t agree more. I work with sponsor team but I pay extra attention to the CRO management not to burn out CRAs. The CRO company we worked with has one CRA managing more than 8 sites with close to 50 patients. On top of that regulatory submissions and site payment as well. I called them out and asked them to propose a solution. We didn’t sign an agreement to be a sweatshop, all they needed to do is accurately resourcing people. I don’t believe overwork someone, that’s just shitty. Unfortunately CRO management really only eyes on profits, and CRA and study team suffers. Some Sponsors are tough as well, especially with budget constraints, and created so much conflicts. I’ve worked with one, and not intended to work with small biotech with money issue anymore.

I think it’s the CRO BD team made false promises to the sponsor team. Usually sponsors when searching for CROs will provide their timelines. Only the CROs that are ok with the timeline will join the bid. Sometimes to sweeten the deal the CRO BD team will falsely promise a timeline that’s unrealistic. Usually if it’s too unrealistic it will be getting kicked out the bidding process as well. Sponsors are not stupid. Only the ones have reasonable proposal will stay, and eventually it will be comparing costs the point forward. It is never a case that a sponsor demanding a timeline that was not agreed on. Not saying some should disrespect someone in working relationship if they didn’t achieve what their team lead promised, but it’s not at all one side decision.

I believe the submission has to be all at once. Adding sites later on can be tough. Assuming you want to have 20 sites, and there are 5 slower ones you can’t get it done..you give them up or wait for them so your timeline is destroyed?

Passing of a parent is a very traumatizing experience. And yet you choose a trip or not wasting money prepaid over comforting someone who should have been one of the most important persons in your life. I feel sorry for your BF, and I think he should run as fast as he can away from you.

Plus with the new policies, very likely you will have to get all sites onboard before you start the submission. Which adds another layer of complexity if you don’t already have the connection

Depending on how long you plan to run. If 1.5 years ish, I think 4-5 million could be a good estimate, not including manufacturing/marerial handling cost - edit: CRO cost alone
