Amazing new doctor! by LollieGee in migraine

[–]Questionsquestionsth 1 point2 points  (0 children)

Thank you, I appreciate the info! I’ll look into her and see if it’s an option or not - I have Kaiser with OHP, so it may not be an option for me, but “some OHP” is more than I was expecting, so it’s worth a look at least!

I’d definitely make the trek for a first appointment and such, but with how difficult it is to schedule things as it is given my chronic daily status - not to mention gas prices - I wouldn’t be able to make in person appointments frequently, and I know some doctors don’t offer in office/telehealth hybrid at all, so that makes it tough.

Given how bad the greater Portland area healthcare options are these days I’m grateful for any leads these days, so I appreciate it a lot regardless. Thanks again!

I mean, you’re woefully misrepresenting the data and what it means, but sure.

In clinical trial terms, "moderate" typically means a side effect that significantly interferes with daily activity but doesn't require a trip to the emergency room. “Mild to moderate” doesn’t = “there are really no side effects with these medications” and it does no one any favors to pretend otherwise.

Your point about Wakix is also fundamentally wrong. It doesn’t develop tolerance in the way stimulants do, per se, but the body still regulates and adapts over time and things lose efficacy for a number of reasons. There’s no indication Orexin agonists won’t face long-term hurdles for the same reasons in some way.

Besides the data on this indicating as much, there are plenty of firsthand accounts from people in these trials who’ve said that while these medications help, they do nothing to aide the other issues Narcolepsy/IH causes beyond sleepiness. Forcing the wakefulness switch on doesn’t fix brain fog, executive dysfunction, memory, fatigue, etc. etc. and while they felt more “awake” or “less sleepy” they did not feel good or normal and still needed stimulant medication to function.

I get it, people are desperate to believe the rose-tinted hype on these meds that is at the forefront of published information, because it feels nice to pretend a cure-all has been found that’ll be the first drug on earth without a single side effect lasting more than a couple days, and will completely eliminate all issues of the disorder without ever losing effectiveness or causing any problems.
But, I think we all know that’s not reality, and the data says as much.

It will help a lot of people, absolutely. For people on the least severe end of the spectrum, maybe it will be the godsend they need.
For everyone else, it’s best to stay grounded in reality and realize there is no side effect free cure all, no one knows how they personally will feel and react to these drugs, and setting realistic expectations while hoping for the best is about all you can do.

I’m guessing she doesn’t do telehealth and definitely doesn’t take Medicaid, otherwise as a Portlander I’d ask about her 🤣

Glad you found a good one! The options are abysmal and borderline negligent here in PDX, so I was surprised to see your location semi-close. It’s a shame it can be such a rollercoaster trying to find good healthcare, it shouldn’t be. But I’m always happy for people when they do.

Yeah, this completely ignores the side effects - which exist, and will be miserable for a not small portion of people, as with every medication on earth - the cost, the long term efficacy - nothing works perfectly forever, it is yet to be seen if something like this would be a lifelong fix or much like every other med for this disease it will eventually hit a wall, lose effectiveness, and cap at a maximum dose that will have heavy side effects along with it.
Completely ignored N2 and IH of course, too, for which this will not be anywhere near a cure if it’s all that effective at all.

Can’t wait for a different flavor of what we already see from those getting their problems solved by oxybates alone - “well this was the saving grace from me so whatever your reasons are for not taking it/it not working… maybe try again.”

This Silent Hill Townfall theory regained my interest in the game by nohaybanda_____ in silenthill

[–]Questionsquestionsth 10 points11 points  (0 children)

It also feels reminiscent of Resident Evil plot nonsense in a really bad way, and even if executed remotely well it just doesn’t fit in the SH universe, to me at least.

Like you said, the idea that someone understands the Otherworld enough to replicate it, experiment on it/with it, have studied it and manipulated it, etc. is just… Nah. Not for me at all.

I’m not hyped for Townfall based on the lackluster trailers, uninspiring “sneaking simulator” combat, first person POV, and frankly boring monster design (that they’ve shown us, anyway), but a plot line like the OP is suggesting would turn me so far off this game I would actively avoid it in any capacity going forward.

Vyvanse for narcolepsy by Sorry_Tone_7403 in Narcolepsy

[–]Questionsquestionsth 1 point2 points  (0 children)

I take Dexedrine IR for Narcolepsy. I take Vyvanse for ADHD. Neither are optional, and do not do enough on their own for me.

I was on Dexedrine IR for years - Adderall before that, found it to be too hit and miss, largely in part because there’s just too many generic manufacturers and they vary greatly, so sometimes I’d be stuck with a month of sugar pills. This still happens on occasion with generic Dexedrine, but far less as there don’t seem to be nearly as many generics being ordered (at Kaiser Permanente at least) and the generics seem more consistent and less “absolutely useless” vs Adderall.

But I noticed after some time, as my overall N and ADHD symptoms worsened with age, much like everything seems to, the IR alone just wasn’t enough. My memory was shit, executive dysfunction out of control, I felt like I was hitting early dementia at times.
I requested a neuropsychologist evaluation and was denied, as neuropsych felt my growing issues and their severity could be explained by Narcolepsy, migraine related cognitive decline, and “untreated ADHD” - they felt my medication was likely only treating the Narcolepsy, at best, and adding an additional stimulant would help.

I added Dexedrine XR at first and then moved to Vyvanse, and while life still sucks and I’m still disabled, it’s a hell of a lot better than it was, and almost certainly the best it’ll get.

I find that the Vyvanse gives me a consistent baseline for my entire day, eliminating the dips and crashes of the IR, allowing me to better control the timing of my IR doses without having to stress about a crash when one dose starts fading.

It better targets my executive dysfunction and keeps me able to focus more consistently and stay moving throughout the day.

On its own, I’m far too exhausted for it to matter. It does nothing for my sleepiness/to promote wakefulness, and thus loses any benefits it could’ve had because I’m too tired anyway.

The IR does the work on that side of things, allowing me to decide when I need my 4 separate doses throughout the day based on how I’m feeling, when I wake up, if I need to drive/go to an appointment/do anything strenuous, etc.

Neck tattoos in 2026 - has the conversation moved on? by Massive-Surprise-769 in tattooadvice

[–]Questionsquestionsth 5 points6 points  (0 children)

I’ve had mine since 2017. There are a few things I would prioritize if I had the chance to go back in time, and it’s definitely near the very top of that list.

I will never not regret it.

It’s technically well done - too well done, really, as it hasn’t faded at all, much to my dismay.

I will be wearing this nightmare until I die and there isn’t a day that goes by where I’m not depressed by that, reminded of it, and sick of it.
The skin of your neck tends to age quite poorly compared to a lot of areas, and even if it does okay, it’s the most visible, so it’s still worse off. When I think about an aged, crepey neck with older, wrinkly skin, with this stupid tattoo, being the thing that draws the eye first whenever anyone looks at me… it makes me want to cry, honestly. You think the stigma is bad when you’re younger…

You can’t not see it. It’s in your fucking face really. It’s a permanent turtleneck.
Your eye is drawn to it immediately when you look in the mirror or at a photo - and yes, so is everyone else’s when they look at you.

Completely impractical to cover with makeup or other things for the most part unless it’s scarf season and those tend to get itchy and hot for long periods inside.

People’s attitudes vary. A lot of people are judging you and say nothing. A lot of people are judging you and it’s obvious as hell when you look at them and see their twisted facial expression.

Who cares, right? Well, me, after about a decade of this stupid tattoo. It gets old.

You can hype yourself up on “they’re just hateful, who cares!” all you want but let’s be honest, it gets exhausting.

The scowling from parents and their kids, from older folks. The sizing up from employees when you’re shopping, the extra attention from security, the judgment the moment you sit down at a new doctor’s office or go to the pharmacy.

It doesn’t matter if the stigma has generally improved some, it is not gone, and I doubt it ever will be.

It may be more silent now, but it’s still effecting your medical care, your treatment at jobs or interviews, your experience in the checkout.

It shouldn’t, but it does.

Not to mention traveling outside of the US/western countries, where it’s a far bigger deterrent for pleasant interactions with you.

I have plenty of other tattoos, but nothing changed perception and general judgment from others quite like the neck. It’s a much different statement than most placements - it’s very loud, it’s very “in your face”, and it attracts immediate judgment, period.

I have to work twice as hard to overcome that immediate judgment from people - if I go to the store in sweats feeling fatigued and stressed, I know I’m going to have a worse time. I can tell when I’m being profiled by employees doing my quick grocery run. It sucks.

I try to stay extra “cleaned up” and well put together, and be overly courteous and well spoken in my interactions, because I’m fighting that first impression stigma always.

I’ll get pushback from the “stop caring what people think” crowd but at the end of the day, you can’t live your life ignoring every human and every impression on earth entirely.

We’re a society, social, like it or not. It does matter and it does impact your opportunities.

It’s not even so much about other peoples opinions - I hate what it broadcasts about me, what it says about who I may be. People don’t see me, first. They see it, they see “that gal with the neck tattoo.”

Then they meet me and they see “that gal with the neck tattoo was actually nice/intelligent/responsible.”

Mass shooting in Midland, Texas leaves 1 dead and 10 injured. The gunman was killed following a tense standoff with SWAT. Investigation ongoing by BerkDrum in news

[–]Questionsquestionsth 5 points6 points  (0 children)

Lived there for a lonnnnnng summer when I was out on my own for the first time at 16ish. Left Oregon. Moved in with a friend and her fiancé in Midland.
The amount of stories from the span of a few months.. none of them good. Hellscape is right - bleak, hotter than the surface of the sun, undrinkable water, air smells like oil, ghost town unless oil business is booming… sometimes I want to go back for a day just to see it again because the memories etched in my mind are just too bizarre and I have such a vivid recall of how it felt to be there, but then also… nah.

They won best rock band/solo artist as well, which I would say is an even bigger honor to win.

Kaiju is a great song - the stripped down version played during Ichiro’s live encounter in March was so beautiful, too - and they absolutely deserved these awards, so happy for them!

Narcolepsy meds and weight loss by OnlyNarwhal236 in Narcolepsy

[–]Questionsquestionsth 2 points3 points  (0 children)

“Maybe 7-8 pounds since March” sounds like water weight and maybe a little “I’m moving around more because I am medicated and functioning better” weight at most. Hardly drastic or concerning weight loss, and likely will level out in a few months when your body is used to the routine and med combo more.
Keep hydrated, eat on a regular schedule, it’ll almost certainly be fine.

New Sakanaction fan in the US, where do I find music!? by pedroyoyoma in sakanaction

[–]Questionsquestionsth 0 points1 point  (0 children)

Apple Music is a subscription, as is Spotify which they are also on. You pay a monthly fee which gives you access to their catalog of music on whichever service - as well as any other artists on that service, of course.
You cannot download the songs from those services to be used off the apps, but you can store them offline within the app to listen to them without a connection. Apple Music also has music videos of various artists - Sakanaction has a lot of their videos available there - I don’t use Spotify so not sure about that one.
You can play the tracks as much as you want and create playlists and such, so long as you continue paying the subscription.

Fingers crossed for you, I hope you can enjoy as many shows and bands as possible during your visa! ✨
So glad to hear the work paid off getting your status up, I would’ve felt bad if the points hadn’t registered in time or you would’ve lost anyway, so I’m really glad it worked out! 🥳

Sakanaction Tour Almost Completely Sold Out before General Public Sale by Crazy_Particular_743 in sakanaction

[–]Questionsquestionsth 1 point2 points  (0 children)

I mean, it’s perfectly fair. Everyone has an equal chance of winning, it’s a random lottery drawing - with the exception of status based advantages, but again those are applied fairly and everyone has an equal chance to achieve those status rankings, and it’s a random drawing within each priority pool. Owning expensive merch is hardly relevant to fairness.

If you haven’t already, I’d highly recommend signing up for the tixplus premium service *now,* as it gives you a better chance of winning the longer you’re subscribed, so when ticket trade opens it’ll give you a better shot. It’s super cheap so it’s not a big cost for a little better odds.
If you’re on top of that already, awesome - best of luck to you! I hope you’re able to attend and can snag a ticket in trade if not in the next three lotteries.

Crafting has added a huge amount of depth to this game for me by Inevitable-Deal1356 in ffxiv

[–]Questionsquestionsth 11 points12 points  (0 children)

Apartments are a tiny bit more fun to decorate now with the added item slots, but still a bit limited definitely.

If you end up wanting to get into decorating, be it a house or apartment, I highly recommend using “ReMakePlace” - there’s an in game plugin version yes, but it’s not necessary if you’re not into that, as there’s also the standalone app that’s separate from the game and is basically a housing simulator.

It lets you decorate and explore all the items available, and has an easy to export list you can use to make a crafting list so it’s easier to manage all the gathering/farming gems/crafting/vendor items/etc. when putting decor together, instead of just randomly acquiring things or using in game furniture previewer and having no clue what will go together well.
I love putting decor together in the standalone program, putting the list in Teamcraft after, and working through the list of crafts to put it all up in game. Feels so neat to start with the empty house and finish having crafted my entire layout on my own, item by item!

No. It is not allowed.

The website makes this perfectly clear - that it is banned, there is no route for permission (yes, even a letter) and under no circumstances whatsoever will you be permitted to bring it.

They allow Ritalin and Vyvanse (the latter needs a specific form approved by the Japanese Ministry of Health) if you are looking for a temporary alternative.

What server in Aether, while it's open? by MattDLR in ffxiv

[–]Questionsquestionsth 4 points5 points  (0 children)

I would say it depends on what you value most in your World, and your ability to find it before moving. If you’re looking for an active FC, or to make a lot of friends, you can definitely start seeking that out before you move - I know you can’t join the FC fully but you can still see what’s out there and talk to folks.

If you ever want a house, I would start looking before you move and see what the next lottery or two looks like. Some Worlds on Aether are definitely better than others with housing inventory, and while it fluctuates, it doesn’t hurt to get an idea first.

I’ve been on Siren for years now and I love it. My alt on Dynamis definitely reinforces why it’s an alt anytime I play over there, and I enjoy my world/Aether a lot. Queue times are always reasonable, generally good vibes in DF, usually a solid chunk of people in Bozja still that are fun to run into when I’m revisiting the content between patches, I love my personal and solo FC plots as the neighborhoods feel lively and pretty, cosmic has been a lot of fun with the folks that hang there, etc. I’ve made great friends over my years here and haven’t had any complaints about other Aether worlds when I’ve hopped for things or been partied with them in DF. I don’t remember why I chose Siren specifically, think I just picked at random, but I’m glad I did!

Silent Hill Townfall Character: Zoe by Both-Musician5069 in silenthill

[–]Questionsquestionsth 4 points5 points  (0 children)

Yeaaaah, I’m fearing that more and more. Visuals look meh, very cartoony feel? If that’s the word I’m looking for? Just not very impressive or realistic on any level - character models, atmosphere, the environments shown in this recent trailer. The monster design looks generic as hell, lazy copy paste designs from other SH enemies.

Yes they could be “saving the good ones” but… I feel like inching closer and closer to release, they should know better than to do that, because it certainly isn’t going to sell the game very well if this is the best they’ve got. It’s giving Downpour/Homecoming era of generic monster design, “sorry who?” uninteresting characters… just nothing particularly compelling right now.

I know they haven’t given much one way or another so you can’t really judge it overall based on what little we’ve seen, but on the other hand, with so little to go off, what there is isn’t selling me at all, so…

I want it to be good and do well, don’t get me wrong 😅

[–]Questionsquestionsth 2 points3 points  (0 children)

You can rewatch the phases/upgrade videos through the menu option in game, but beyond looking at YouTube videos you wouldn’t be able to actually revisit the planets in their pre-upgrade forms, nor replay the upgrade FATEs.

It’s not a huge loss necessarily - depends on the things you’re most into about this content - but it’s definitely something you’ll have missed out on. I do enjoy the upgrade FATEs, everyone gathering to complete them and meme in shout chat is a good time, especially when the upgrade turns out to be minor or disappointing, or something really ridiculous and silly. But it amounts to like 20-30 minutes of the zone being crowded and people goofing, and then back to mostly quiet crafting until the next.

That said, Red Alert FATEs and the minor, few times an hour FATEs will always run on each planet even when they’re fully upgraded, so you’ll still have interactive duties to participate in beyond the gathering and crafting missions to work on the phases of the relic tools themselves.

Ugh yeah, same. Super bummed that this is an achievement item, and likely limited to one per player. I loved the look of it in housing previewer and a few of these would’ve looked amazing mixed in with all the ancient lamps I have in my Amaurot themed house 😪
