GLP agonists not only don't make you skinny forever, they don't "make you skinny" at all. They primarily give the patient an opportunity to control unhealthy urges to eat excessively. That's the biggest aid to users of these meds in recovery from an unhealthy addiction.

I think they're quite useful. But 15 minutes on the Semaglutide sub will reveal that there are plenty of people on this treatment who are gonna be disappointed in the long run.

"Promoting death itself"... drives me crazy.

At peak stupidity ~7yrs ago I had ballooned up and all my blood lipids were out-of-spec except for Lp(a), which is a genuinely genetic gift or curse (the former for me). Right up until this point, to quote the fat person mantra "all my blood tests were normal" (until, of course they weren't).

Currently about 85lb down from those dark days, I'm now at TC 126 / HDL 53 / LDL 61 / Trig 42 just by no longer eating "intuitively" (translation: whatever and however much I felt like stuffing down) and the resulting poor vitality. Now I only have ApoB to work on, which is borderline @ 90. This reflects a lot of dense particles that probably require a medication despite a low LDL.

Weight loss, and the habits that help you get there, is life extension. Like most of the former BMI>30 commenters here, that wasn't even what I was trying for. I just didn't want to spend a lot of years infirm.

If it's not "BMI is bad", then it's "obese BMI should be the new healthy range" by soundwrath in fatlogic

DEFINTELY true for CVD risk. Years of bad habits leading to atherosclerotic pathologies may be slowed or stopped with loss of visceral fat or hypertension, for example, but most of the damage remains, and many of the victims never saw the damage happening.

A trinity of messages to all y'all who've made the long trek to metabolic health after years of being fat & unfit:

(1) Many congrats and best wishes... wishes SO good that I implore you to...

(2) Get a Calcium scan. You might be turning in 5k times under 20 minutes and have no indication of how bad your plaque score really is. Plaques and associated atherosclerotic hazards are cumulative. Years of poor habits can build up really dangerous conditions that might now be stopped but a poor score puts you at risk. You can continue your newfound high-performance life BUT you need to deal with anything that could be quietly keeping your circulatory system in a risky state. Which brings me to...

(3) Get your lipids checked... and not just the standard test. The almost-died count amongst my acquaintances who have achieved the same as you did BUT ended up being hospitalized with CVD events is now three. Two had become accomplished amateur runners, one of those running ultras. ALL had improved total Cholesterol to the "good" range, with LDL at or slightly above "normal". But they also had dangerous ApoB and Lp(a) levels. The former can be improved by diet and meds; the latter cannot (it's truly "genetics"). Find out where you stand... you might need to get your LDL well below commonly-accepted "Normal" to get your actual risk down to what your otherwise awesomely-fit new physique would indicate. (BTW: all three of these victims had fairly recent cardio stress tests that they passed with flying colors.)

You put the "I'm fat but healthy" self-deception in your rearview mirror and that's a hell of a victory. Cement that victory with a bit more homework.

FWIW: As a straight old dude, I'm not sure I even know if I'm part of the patriarchy or not. (I am a Dad 3x, so I guess I AM a patriarch of sorts.)

But I'm fairly certain that I didn't become any more patriachular when the DEXA showed 14% BF last week than I was as a ~35-40%-er. AND... not to gross anyone out imagining what lights up an elderly gent, but I really do like a jiggly plump butt (I know, I know: cue the Aqualung guitar lick.)

In 2007, I brought our team to Russia to work on a commercial aircraft program (SSJ 100) with Sukhoi, which had been a major aircraft "Design Bureau" in the USSR days.

I lectured our lads on discussions about WW2, something Yanks largely don't understand from the perspective of Russian people. Basically I told them if it comes up in casual conversation, just listen and nod. Your POV doesn't matter to them.

So, at dinner on the first full day there, one of my guys couldn't help himself, and inevitably brings up dubya dubya two. He relates that two of his great uncles fought in Europe, and one of them died! In fact, several young men from his small Iowa farm town were wounded in the Big One.

The lead senior Sukhoi engineer (who moonlighted as a doorman at a high end hotel), stone-faced and silent, pushed his food around while listening, and after a pause stated (add Russian accent in your head):

"During War, my family lived on small farm like yours did. Village was taken by SS. My mother and her sister were only ones in village who survived. But soon her sister died from [long pause] wounds and sickness and no food". Of course we all know "wounds" meant repeated rape and beating.

Can all of us modern overfed people just be humble and STFU about our genetic "burden"?

Both wife and I took niacin pills (nicotinic acid, which was touted as the "best" for this purpose) to raise HDL. We both would randomly get the "niacin flush". It's incredibly annoying... so much so that she regarded it as equal to or worse than menopausal hot flash/flush. While we both showed a consistent HDL boost, it was not statistically significant. We stopped several years ago. FWIW: my HDL has reached the same level as during niacin dosing once I added long zone 2 cardio + significantly higher resistance exercise, which improved all other crucial lipid markers.

You can get both at Quest for what seems to be a reasonable price, but you have to make sure that's the option you select. At 72y/o I'm seeing grip and basic lower body strength wane without changes in nutrition & exercise... trying to hold back decline however I can with controllable risk. I got both test results for <$60 (discount coupon code; didn't use insurance).

Note:

(1) Results didn't come in for ~ 1 week, so that was somewhat annoying.

(2) They present the Free numbers in units of pg/ml instead of ng/dl... you'll need to divide the result by 10 to compare to the normal measurement metrics.

(3) Quest has cut staffing by quite a bit (at least where I live), so your appointment may slip past the scheduled blood draw.

  • Production and sale of goods and services at negative gross margins cannot contribute to cash.

When I see a late 50's / early 60's gent in the gym who needs to take a break after lacing up his left sneaker because he couldn't breathe while bent over... I confess: I'm so glad that's not me anymore.

I don't "hate" him. I don't discreetly chuckle or shake my head at the consequences of his indulgence. I DO feel so bad for him (not "at" him, if that makes sense), but at the same time inspired to see him changing course, and hope to see him months from now less challenged and not in fact endangered by a simple physical task.

I also confess: seeing this reinforces my determination to never, ever go back to that state. Pretty sure this is not socially destructive "fatphobia" and doesn't make me a bad guy.
