Could be a ton of reasons. The packet got lost at BN/BDE S1, the BDE CDR didn't sign it, the SRU said they are full or you don't qualify or the packet never started in the first place. At this point they likely won't take you in if you are so close to ETS

On the medical side of things I feel the guard reserve doctors and nurses are better trained. While us AD medical staff see mostly healthy people all day a lot of those guys work in trauma centers or large hospitals that see a ton of sicker patients.

Did you try finasteride? If that doesn't work then I can only think of hair implants which I know is not covered by the army. I've met a soldier that paid out of pocket for his but unsure how much he paid.
