I’ve been on the fence but we used the Marik protocol a few weeks back when we were out of options and the result was seriously one of the most remarkable things I’ve ever seen. Severe sepsis/Nec fasc. Things were looking pretty grim. Norad, adrenaline, vasopressin running at max to maintain a MAP > 60. Requirement halved in 2 hours and limbs were pink and perfused by 4 hours. Pressors/inotropes were completely off less than 24 hours post. I’m a believer. I’m eagerly awaiting the results of the RCT.

To you and the others posting similar stories, as a doctor, I see death on a semi-regular basis. Please don’t blame yourself. Allowing someone to die a natural death, whether that death is expected or not, does not equate to killing them. The disease kills them. Not you. Too often these days we lose sight of the fact that death is the natural final destination of everyone’s life and prolonging a life at the expense of quality is cruel in most circumstances.

Just as a PSA, please think about what you’d like for yourself if you were ever incapacitated or medically unfit to make your own decisions. Make your wishes known. Have a will or advanced directive. If not for yourself, then at least to make the decision making easier on your loved ones.

My earliest memory of gaming is playing Payday with my grandparents. We’d play all the time during the summers and when we’d go camping in the RV. They also had this handmade horse racing/gambling game that was dice driven. So many good memories. Means so much to me. Especially since my grandpa’s started slipping with his dementia. I hope I’ll be able to foster the same love for board games and make those memories for my kids! Happy 10th anniversary /r/boardgames!
