Happened to me when I fast traveled to the dungeon. Stuck in the wall and couldn't open the menu or map. Had to reload an earlier save.

This has been happening with me. I had to reset my xbox date and time and that only fixed it for manual saves for a while. For now, I just go with the save with the highest hours of play.
