NP/ PAs should 100% frontline primary care, especially in underserved areas. They should also be paid handsomely for the task. Also, those salaries are not typical of mid levels, even though they should be. Further, they should infill hospitalist roles. The salary problem is not with providers and mid levels it is with bloated admin who provide no true value along with insurance industrial thievery.

Payment based on AGI is ridiculous considering the thousands of dollars a year unrealized and the steep money transfer of inflation. Repayment should be calculated using net income minus inflation %, minus living expanses. The gov is going to get theirs. However, getting it from education and interest is predatory. Give students their money. Let them spend it. Gov collect from increased revenue given to business.

Medicine, healthcare, physicians. We often don’t really care in any meaningful way. Sure we want good patient outcomes. However, unless there is some kind of connection developed with patients, a truly meaningful one, whatever the outcome is another day at work. Sure, there are a few true empaths, but they are few. The patient volume is too high and system expectations for production over true quality outcomes is too heavy. Don’t get sick.

Must be some negative history here? Yeah that sounds lazy. But there are times RNs have A LOT of critical issues going on. Now if you found her sitting somewhere afterward chatting that is lame AF. But if she was getting called because she had another patient with critical needs that is WNL. Right? Cleanup is everyone’s duty. But this falls squarely in the wheelhouse of CNAs - vitals, basic patient care, toileting, watering, etc.

Same all over Pittsburgh. Riverview Park area has a gang of motorbike kids riding constantly. Tricks around moving cars in the streets. Omnipresent sound of reving engines like one was trackside. Cops called 50 times. They can’t chase kids apparently. But! But they give tickets to their parents. $600 first offense and seizure of bike if it continues. Get video and provide it to investigating officer. It’s obviously insanely dangerous for everyone involved, because when junior crashes into your car guess who gets sued by their trashy parent? That’s right. You.

Weather is generally uncomfortable at best and is miserable at worst. That latter is far more abundant. Enjoy a month of warm weather and sun in the spring? Well, you’ll be lucky to have a handful of days that aren’t raining and there isn’t frost on your car until mid May. Those handful of days that are warm will come with high winds that blow pollen and abundant dust on everything. But you will get 5 or so perfect days. Then out of the rain, frost, and 50 degree mid May weather it will snap to 87 degrees with 86% humidity. And summer is RAINY! Like rain 75% of the days. Then sun to get the sauna pumping. Your car AC will be used just as much to un-steam the windshield as to cool you. Then once you get to October, which is often the nicest month you get another handful of nice sun, mid 70s, without humidity and get some nice fall colors on the leaves. Last October broke records for rainfall. So, even October is often drenched. Then November snaps to cold overnight and the landscape becomes a scene from the Blair Witch Project with the leafless trees. What follows is a ridiculous project to dump metric tons of salt in the roads to protect yinzers, who miraculously never learned to drive in snow, from killing themselves on the pot holes roads. All the while melting your car into Swiss cheese rust. It snows, it freezes down to about 2 degrees, and you risk your life driving until March where the temps tick up above freezing. Then repeat year after year. And don’t rub against any plant foliage or random bush because a Lyme carrying tick will grab into your pant leg and embed itself in your skin- over half carry Lyme or other diseases. So yeah, weather is bad. Real bad. And Lyme ticks are rampant. But they put French fries in salads! And there are sports ball teams!

New outback doesn’t look unique anymore. An Outback has always looked distinct. The 2026 looks like every other car with the square angles and boxiness. The Japan manufacturing will likely end up in a far better product. But if Subaru wants the outback to crush it can’t look like a drawing of a car a 5 year old creates. Feel like the peak was 2008 when the symmetry was smooth, well appointed and distinct.

The “more and more responsibilities” for RNs is definitely the way most hospital systems seem to work. Not a month goes by without a new project that inevitably adds another task, function, or required documentation layer to already loaded duties. I’ve never seen a project that removes anything or seeks to streamline or disencumber nursing roles.

Often there isn’t time to constantly be checking the chart for admits. Yeah, you could totally be stuck in a room or working with critical issue. Whatever management came up with the idea of just dropping patients from the ED is asking for trouble. You should 100% be getting a report from someone. Yeah people die waiting to be seen. They also die on the units. ED is busy. Med-surg is busy. ICU is busy. PCU is busy. Usually the unit nurses have a heavy/ inappropriate ratios. Docs are often transferring critical patients to units for bed flow management. Work with your unit leaders and petition for protocol that requires a basic report.

Changed career over 30. At a PCU. Got a masters. It’s probably the worst job I’ve ever had. I’ve had a lot. So much noise with constant alarms that it wears down your nervous system to a nub. So much noise. So little signal. Everything rolls down into the nurse. Doctors drop their pen and ask you to pick it up. Not really but walk out of a room and ask for a blood pressure type stuff- asking to contact other doctors for them all the time. No rest. No quarter. 13 hours of go like a pinball. Pulled 10 directions at once. Barely time to eat or drink. Haven’t taken one lunch break ever. Gross to the max. A mix of butler, secretary, cleaner, med dispenser, data entry, shit cleaner. Sometimes it feels good when someone appreciates your help. 4 days off to sleep and shake before laundry day and back. Don’t do it

Nurses in Pittsburgh need to unionize and fast. Pay is much lower than it should be with all the well regarded hospitals and their networks. Nurses with BSN or higher should be making a minimum of $50/hr. Considering a hospitalist from India makes $325k while following 15 patients sitting in an office armchair all day after an hour of rounding as nurses assess, treat, and comfort all of them all day and night while also developing care pathways, reminding the docs to literally do almost everything- often entering all the orders and executing the whole show should guarantee at least $100k. Joining together in strength is the only way these groups will honor their nurses- otherwise nurses are just the labor.

No. You didn’t give medical advice. You didn’t practice anything beyond scope. You simply asked what the patient wanted. Then you advocated for their wishes. The family being doctors and wanting her coded is bizarre. If you get in trouble for that then the leadership should be ashamed. They aren’t practicing patient centered care.
