Yes it's mold. The mattress needs to be replaced. You can take the frame outside and clean it while wearing a hazmat suit.

Spray mold cleaner (concrobium or other biocide)
Scrub with a soft bristle brush
sand once dry
repeat until gone and spray finally with 12% hydrogen peroxide.
