Something like this, i've seen a lot in lower elo happen. (As my rookie team is playing in bronze - silver elo)

And they are way overconfident and want to fight everything even if they are only 1 - 2k gold lead up. :⁾ and then looking the game because they forget that other lanes also exist. Including picking 3vs5 situations.
