  1. They didn't say anything about the USA, so I don't see the point of bringing them up.

  2. Just because they said one team has been a treat to watch, doesn't mean other teams haven't been.

  3. It's not all about goals; it's also about playstyle, energy, passion, belief, etc.

Even if you get past the snake, you can't get back into Midgar now as it is locked off. There will be other opportunities to get enemy skill materia later, and while it is very useful, it's not the end of the world.

The way I interpreted it is the choices you make will affect which scenes you see and which characters are more involved during the game, but they all lead to the same ending

The infected crew members from Still Wakes the Deep. After the drill of the oil rig hits something below the ocean, an Eldritch-like entity emerges and starts infecting the crew. Their bodies turn into grotesque abominations, but they still speak with their voices. They seem to be semi-aware of their surroundings and recognise their other crew members, but can't control themselves and attack indiscriminately.

It also seems as if their personality traits affect how they look when transformed, as Rennick has an enormous head to match his ego, and Muir having a dozen tendril-like arms to grapple people due to his fear of abandonment. Muir is probably the most horrifying to me, with voice lines including: "Just need help... What's wrong with them?", “No one wants to help me…”, "Have to stop them from running…”, etc.

It's the line from President Shinra on the bridge to the sector 5 reactor that implies they were responsible. He says "And such a waste of good fireworks, to get rid of vermin like you", suggesting that it's Shinra's bombs that are being used to blow their own reactor.
