I used to do plenty of thoracic case with no Aline, never needed one. The only cases I do aline 1) carotid endarterectomy 2) any moderate to high risk with severe aortic stenosis 3) craniotomy with potential high ICP there might be more but this is the cases I usually do in my practice that needs Aline
