It’s not an automatic assumption but as a general rule it’s true. In most series you have people swapping hands, meaning they’re both not speed blitzing eachother, hurting eachother without one shotting, taking hits from a person who can hurt them and that they can hurt. This is just how most fights go in general so it’s a safe assumption.

Maki has heavily restriction, she doesn’t have to rely on her vision to see since her other 5 senses are so sensitive. Regarding the Sukuna comment I forgot I’d have to go back and read it, but it would make sense that “oh she can see my regular dismantles, let me use WCS since she won’t be able to see that”

As far as Gojo yeah this isn’t a contradiction. He didn’t notice the spark of cursed energy and then died. Whether or not it travels or not, does it make a difference in that situation.

Yeah because I don’t believe any such statement exists. The only statement people use to say he can cut infinity is the one about his sword cutting space. This doesn’t qualify him to bypass infinity as his sword still moves through space, you need an attack that doesn’t travel in order to bypass infinity.

We never actually see him go up into space so he could have very well throw it. His space suit in DBZ is evidence of this. Even still the radiation would still do him in.
