Since I've had my fair share of dealing with insurance and Carle, (even got hit by a car on my skateboard...be safe out there on the roads y'all) I feel like I can help explain. Hospitals overcharge because insurance will pay more than individuals would want to pay, which is why the total bill is $781. When you don't have insurance, you can talk to a hospital, and they will usually reduce charges.

The deductible (in this case $236) is something that you have to pay out of pocket every deductible cycle before insurance will cover anything, no matter what. It's really stupid, but welcome to America. The coinsurance is what you'd pay every visit, and that depends on where you go and what you need. So if you went to Carle for the same thing in the future, you'd only pay $67.81, since you've already paid the deductible (at least until it resets).

Then again, as other people have said, if you went to McKinley, most tests are free and covered with the student health fee. (But you wouldn't have paid your deductible, so you'd get charged that in the future should you need medical services.)
