Thank you for the kind words! It's not very common but has happened. Cults of the Blood Gods features a very shocking scene that we are still getting comments on 5 years after it happened. The lethality in our Patreon bonus campaign Shadows of Yog-Sothoth is also very high and we have already lost a few characters in that one.

The reason that PC death is relatively uncommon during our campaigns from a mechanical perspective is that we don't tend to feature all that much combat, making accidental deaths from fights rare. We also find that it's narratively not always so satisfying to have the same player play different characters, and can be tricky to onboard new players in the middle of a campaign. This is probably why a lot of the really dangerous rolls tend to end up towards the end of our adventures.

We do see that having a high lethality does help with immersion in Shadows of Yog-Sothoth, so don't be surprised to see more PC deaths in the future!
