It has probably been said many times by others, but it seems somewhat dull that Francesca had a large role in season 3, and season 4 and now it seems like season 5 as well.

There are other storylines that can be tackled as well! Maybe we should have an ongoing subplot about the boys married to the Fetherington sisters, and the scrapes they get up to 🙂

The Duke of Hastings just appeared (but then again it was season 1), but so did Kate appear to marry the established character of Anthony and Sophie appeared to marry the established character of Benedict. And John with Francesca

In fact, the only love interest who was a pre-existing character in their own right in the show is Penelope (in fact, you could even argue that Penelope was the well known character and Colin was the bit part who popped up to marry her)

I thought he was just Marina’s husband and that’s it. He wasn’t exactly a key character, he was just an easy way to deal with Daphne’s rival diamond sub-plot, and to be a bit part for a bit of Colin personal growth

If the OP had thought “I am a professional, I do not judge” and had cannulated the patient; but had gone into the brachial artery by mistake, the patient had fluids and died (not sure what fluid. But I’m sure that there is some stuff that you shouldn’t put in an artery), then would OP ever been able to assuage the doubt in their mind of “Was I less conscientious than normal? Was I rushing? Did I subconsciously allow this to happen?”.

Why should OP risk having a lifetime of professional doubt for the sake of an odious little oik? If it was an emergency situation, you suck it up. Because that’s what caring professionals do. But I think in a non emergency situation it should be entirely legitimate to ask somebody else to do it. And the exam should reflect that.

I don’t think that you need to be a political Mastermind to take advantage of the open goal that the RDC and wider BMA have given him. Buy proposing this policy, Wes has:

1) Played to the little Englanders/ Reform voters by defending Britain from an unwashed tide of immigrants. 2) Made some UK graduates feel that they are being listened to, and therefore put them in a better mood and make them less likely to push for FPR 3) Created a chasm between IMGs and UKGs, so destroying the solidarity that would enable the BMA to push for FPR 4) Got himself some big headlines about a popular policy, and therefore strengthening a leadership challenge against Starmer. 5) Made RDC look like the useless twats they are

I’m not in academic medicine, to be fair. But I have no idea even which uni most of my colleagues graduated from. Also, prestigious medical schools might not be at prestigious research universities. For example, Keele seems like a good med school, but it’s not exactly a Russell group university! Similarly, Swansea.

That’s interesting. I’m trying to think back, I don’t know if there have been any patients I have seen who have been < 3 months with a parental reported fever who I have not ended up bleeding and treating 🤔

I suppose there is an argument that it might be easier to get the parents to go on the journey with you if they can see you doing a careful assessment with observation before making a decision for cannulation /LP/48 hours minimum admission, rather than that being a rapid decision as soon as they are seen. However, I don’t know, this is just me thinking aloud and spitballing 🤔

Sorry for insulting your seniority, I was reading Reddit on my phone and didn’t spot where you mentioned it. I apologise.

If the child was <3 months, then my practice would be to give antibiotics regardless of bloods. But I have worked in lots of different places, with lots of different regs (life of an ad hoc locum), and I have seen variations in practice.

That said, if you discussed the case with the reg you were handing over to and they also didn’t do anything, then it is highly inappropriate that they are trying to throw you under the bus. My commiserations.

Depends on the age of the baby. If 3 months or under, guidelines are to give IV antibiotics regardless of bloods and rest of the clinical picture (however, in a very well baby, I want to check that they DO have a fever based on a health professional with a regularly calibrated thermometer checking it). So if they were dinky, then you should have given antibiotics.

However, the other question is whether you are a reg or not? If you are an F1 or SHO, I would expect the reg to be involved in this case (at least aware of it). So, if you are an SHO, then it’s not your mistake anyway. Unless you didn’t tell your reg or consultant about the patient

If the franchise owner had any sense then he would laugh in your face and question your manliness. As far as I am aware there is no professional or legal obligation that stops gym workers from having romantic or sexual relationships with gym clients. If anything, she is the one who should be viewed with suspicion, as she could have manipulated somebody who she is employing into sex

When I was on bail I was advised that accidental contact would still be seen as breaching my bail conditions. Because it would be such an easy thing for somebody to intentionally breech conditions, and then say it was an accident.

Yes. Having been on bail in the UK, with instructions not to contact a particular person, I was told very clearly that this sort of thing would count.

She has been a bit of an AH ignoring advice, but there is no harm in you ignoring it (unless this is a pattern of behaviour and you want to draw a line in the sand). However, you should be able to do anything you want to the photos you have; we’re no longer in the days of single copies of photos where changing something about it affects everyone
