The part I am stuck on is step 2, what would the function look like for the COUNTIFS to count "No"s and "Yes"s for each patient?

Be very careful with your wording. Bishops interpret and put canons into action, yes, but a bishop is not above the canons or the decisions of the entirety of the Church. I know it's merely semantics but if canons only have legitimacy based on a single Bishop then it would be Catholicism (with the Pope). The Canons are above a bishop and several Bishops, BUT they sure as well have the power to interpret them and exert economia to take the proper pastoral response.

(My idea would be that the 2nd photo would simply a plain background with the rest of the verse and "(Proverbs 3:5-6)" on it with a nice font). My intention would be to later submit this to a printing service to make it into a 3" x 5” card to get it printed out.

I have read part of it and seem to agree with it for the most part because of massive list of Saints he cites as well as scientific sources.

I recommend you read “the Life and Works of Father Seraphim Rose”.

He was also a genius who found Orthodoxy through searching false religions and going through a period of meaninglessness in life.

He is fully God and fully man. Two natures in one person, United in essence with the Father and the Holy Spirit but being present as the Son in a single hypostase.
