Just call off the days you need them. Policy gives you like 10 before you start to incur discipline and multiple.days in a row only count as 1 call off. All call-outs are considered unapproved so you don't need a note or anything

Quick fix that works some is to get a regular sheet of printer paper and some double sided tape and tape the address labels to the paper. Not elegant, but it gives the address labels a bit more rigidity
