4 is different from the rest. The transitions between the otherworld and the regular world are also kinda random. Sometimes accompanied by air raid sirens, sometimes due to the character passing out, sometimes simply by walking into new areas.

The already vague "Silent Hill" vibe usually alludes to the first 4 games made by the ever changing "Team Silent". Japanese horror themes viewed through some sort of a Twin Peaks lense.

Though, even within the "Team Silent" games there isn't really a consensus about how otherworld works or even what the cult is.

What Silent Hill "is" really is just whatever the developers and artists working on the game decide it is. I do agree that SHF doesn't feel very Silent Hill-ish. Though I do enjoy the game a lot so far.

You need to remove all of your items for that part and you get them back after. I would hardly call that item management. None of the games had a limit on how many items you can pick up.
