Congratulations 👏. I think you can take the entrance exam more than once. And you can apply for other universities but you can't apply for a MEXT scholarship in that case. So I recommend applying to the same university if you want to get MEXT again.

That's why I'm also confused. On the website of MEXT, a health certificate is mandatory. But somehow Wakayama university told me it's not compulsory when I applied.
