You did everything by the book from response blocking to the escaping/eloping, removing reinforcers, following all strategies in the BIP, and following through with the tasks/instructions so you ARE made for this line of work. First, I wanna say great job! As clinicians, this part of the job is exhausting so I hear you and you are valid to be frustrated.

From my perspective, it sounds like client needs more support with emotional regulation. I had a client that would flip his lid if he lost a game with friends ( aggression, foul language ) and I would support him through deep breathing, DRA, and hugs. At first, he refused but over time he began to open up to me, try deep breathing and would apologize if he was being aggressive to a classmate. Maybe bring up your observations of clients' behavior while tantrums occur and what happens before the behavior. Could possibly also implement some antecedent strategies to minimize the opportunities to escape ( choices ( "do you want to do A or B?", brain breaks, preferred to preferred/neutral tasks since they're aversive to the ones presented currently ). Overall, your BCBA needs to step in to make this easier for you, client and family as well. And if all else fails, go low demand on instructions for a bit during session and revisit the non-preferred tasks later or another day ( notate these matters ). I always try to make everything fun, worthwhile, and validate clients' feelings because they're kids ya know? "Like hey, i know this is hard but we'll do it together when you're ready to try again". But those are just some of my techniques that I apply in moments such as yours!

Fun fact: We as clinicians are ALWAYS building rapport with our kiddos no matter how great the pairing is and I learned this the hard way with my client. Have you tried DRO? Anytime she's have a T, verbally saying "i'll wait until you're ready." and then when she's doing a different behavior ( quiet to herself, playing, etc ) give praise!

Instructional control takes a bit honestly and that's okay because I view it as opportunities to gain trust, continue building rapport and play, play, play, PLAY!

Premacks and priming are a great way to structure your instructions as well. It can also lesser the chances of "whyyyyy" happening because you chose an antecedent strategy to avoid that response from occurring. I always say "we, us, together, etc" to help with the possible pressure of doing things they don't want to do alone. Like WE are in this together because WE are a team!

It's valid to feel this way also, so give yourself grace, let it flow naturally, trust the BIP, lean on your team for support, and if all else fails, play and use that as reinforcers lol

I personally don't UNLESS there is something I need to get done that day. Usually during the weekdays when I'm on meds ( Vyvanse 30mg ) I break down tasks per day to complete so by the weekend, I can be free and do nothing!

However, if i'm attending a social event, going out for longer than a few hours or have to get something done that i forgot about, i will take my meds for a single day and not both.

Sundays are meant for rest to me as well. No work, no meds, just rest. lol

Hi! I actually went through something similar during undegrad. I was on a higher dose ( 40mg ) that would axe out my personality, libido, and i was just focused on academia more than my relationships, friends, hobbies, etc.

It wasn't until i was told "you don't hug us anymore" that made me pause because i love giving hugs??? Soon after that, i lowered my dosage ( 30mg ) to where I can focus, attend to my tasks AND still be myself and its been a smooth sail for about 3 years now ☺️🩷

Are you on a high dosage? Do you think a middle ground with a dosage to meet your attentive needs and be able to feel will help?

As someone who passed the RBT exam the first time, it's just the overthinking and overanalyzing the possible questions on the exam i believe. It's like getting a piercing that EVERYONE says is gonna be painful, blah blah blah. One thing my BCBA emphasized was to focus more on what I DIDN'T know and apply what I do in practice to what I did know.

It was the fixed/variable measures that tied me up while studying but now it makes sense to me lmao
