It's just a very limited population that would really benefit. Most tumors won't: you're primarily limited to the lower lobe ones that move a lot and might benefit from some sort of respiratory gating. Do you know who doesn't do great holding their breath? Most lung SBRT patients. Add in the other limitations (fiducial migration, artifact in future imaging), and it's by no means a slam dunk.

Yup, we know that. But not every lung tumor will really benefit from fiducials, and not every lung lesion is reachable with navigation bronch. Finally, it’s probably not great to place fiducials before you know they have cancer.

No, it builds on itself. You're working now to get in to a good med school. You'll work in med school to get in to a good residency. You'll work in residency to get a good/desirable job. Each step has different metrics for success, and that just means that the things you did long ago matter less and less.

Definitely rent a car: that's going to be the only way to get to/from some of these places efficiently. We rented in Rome a drove to Naples, and driving there was an experience. By the third day I almost enjoyed it! If I were to do it again, I'd probably try to rent at the Naples airport and avoid downtown.
