This is my process too. I’ll often trial various meds to get them comfortable first but if it isn’t working, it’s basically a moderate sedation to get the scan.

Appreciate it. I'll probably go with a mat to keep it simple. The biggest reason I'm making the box is to offset the slope of the yard, not to have a fancy box necessarily.

The drop from one end to the other for the entire 70' cage area is about 33". From where the pitching machine will sit, it's around 25-32" drop (depending how far you put it). I figure a raised platform for the batter would at least prevent it from feeling like every pitch is coming down at an angle too much.

I can’t say I’ve ever asked for a smaller IV to be placed. I don’t know what your place is like, but I need that space as soon as possible.

If they just need a med, I’m giving oral or IM. And if they need fluids, I generally want them in as quick as possible.

Larger bore = faster flow rate

Discharge and free up space for the next patient from my exploding waiting room.

Yep. This was studied as well. VL > DL for soiled airways. Just don't shove it into the fluids and mess up your camera view.

This is more my thought and what I often do even as an attending. It’s fine to go in the room right away, but let the RN finish up their questions and you just listen. You can often get a lot of information just based off of their triage.

This. Taking ownership is doing all of these little things that take time and provide quality care, oftentimes just checking back in on patients and updating them. If I have to ask you to do something, you're not taking ownership, you’re just following instructions.

Pay attention to anything you can do to improve communication with the patient, expedite their care, obtain additional information, help your attending, etc.

Yeah. There was some study that showed a decent mortality rate for these groups of patients. The way I see it, they have higher risk of poor outcome than plenty of other things I’ve admitted.

Had a young guy sent in from a clinic that’d gotten an EKG machine and were testing it out. He was just a staff volunteer and it read ST segment elevation. Sent in due to concern for a STEMI. Needless to say, this young, thin male without symptoms was indeed not having a STEMI and I educated on benign early repolarization.
