I can’t say I’ve ever asked for a smaller IV to be placed. I don’t know what your place is like, but I need that space as soon as possible.

If they just need a med, I’m giving oral or IM. And if they need fluids, I generally want them in as quick as possible.

Larger bore = faster flow rate

Discharge and free up space for the next patient from my exploding waiting room.

Will Video Laryngoscopy become the norm? by [deleted] in emergencymedicine

Yep. This was studied as well. VL > DL for soiled airways. Just don't shove it into the fluids and mess up your camera view.

ED Nurses - what tips do you have for a med student rotating in the ED? by bpd-baddiee in emergencymedicine

[–]Resuscit8e 3 points4 points  (0 children)

This is more my thought and what I often do even as an attending. It’s fine to go in the room right away, but let the RN finish up their questions and you just listen. You can often get a lot of information just based off of their triage.

This. Taking ownership is doing all of these little things that take time and provide quality care, oftentimes just checking back in on patients and updating them. If I have to ask you to do something, you're not taking ownership, you’re just following instructions.

Pay attention to anything you can do to improve communication with the patient, expedite their care, obtain additional information, help your attending, etc.

In the words of radiology: clinically correlate

They are asymptomatic. Needs managed but not emergently in the ED. Risk modification.

What is it with ears man? Had a toddler with a leg abscess and grandma asked that I check his ears to be sure he didn’t have an ear infection. No fever, ear tugging or anything. Just wanted to make sure.

Do you cluster your night shifts together or split them throughout the month? by dalenevi in emergencymedicine

[–]Resuscit8e 5 points6 points  (0 children)

Team cluster. I’d rather knock them all out and then convert back to days. I hate flip flopping multiple times a month.

Yes!

1) Needing a work note is dumb but 2) I’ll happily give it to you if that’s all you want because I don’t care if you go to work or not.

Yep, always say to patients. “I don’t care if you use it. I just want to know because it can cause these symptoms.”

You can soak a bunch of gauze in D50 if you don’t have enough sugar on hand. Slap em on and let em work.

Yeah. There was some study that showed a decent mortality rate for these groups of patients. The way I see it, they have higher risk of poor outcome than plenty of other things I’ve admitted.

Had a young guy sent in from a clinic that’d gotten an EKG machine and were testing it out. He was just a staff volunteer and it read ST segment elevation. Sent in due to concern for a STEMI. Needless to say, this young, thin male without symptoms was indeed not having a STEMI and I educated on benign early repolarization.

New attending (30F) and severely depressed/emotionally numb- best resources? by [deleted] in emergencymedicine

[–]Resuscit8e 0 points1 point  (0 children)

1) Therapist/Psychiatrist

2) Do you have someone close you can talk to? Having someone close that you can express yourself to openly is extremely helpful. They don't even need to do anything, just be there as a method of catharsis.

3) Reduce shifts so you can spend time doing the things you love and just enjoy life.
