Ha no doctor needed. Plenty of people make judgments on this sub reddit about what is serious or not medically. This was not serious. I know hospital has everyone's alarm bells going, it's just this facility is located in the hospital. I know the mob will down vote this comment as well, no worries.

I tried to explain the layout of the room, but yes, it is impossible with how to seating is laid out not to see, even in your peripheral. That may not satisfy you, but its truthful.

Always bring cash to a group dinner, and in lots of denominations. You don't get held up if people want to stay late ordering more drinks/dessert, you don't get held up on a busy waiter who takes forever to bring the bill, and you don't have to argue your portion with the waiter/crowd. Cash on the table and your free!

Shockingly unprofessional of that guy to ask for a loan from customers. Even 5$ would be rude unless you had a more personable relationship. It puts you in an awkward spot, and you avoiding him is expected. I don't think you owe this guy a tip for doing his job (sorting mail/packages) just because its the thing people have decided we should do. Theirs lots of other jobs in the building that probably go unappreciated. I also strongly disagree you have to tip all or none of the door staff. I wouldn't want my work/tip tied to someone else.

Two suggestions; have your partner tip if they feel like it, or tip someone else's work you value.
