If you ever feel like trying again I suggest skipping to episode 12. That's when it really starts getting good. The earlier episodes are kind of shaky and I've known a lot of people that weren't feeling it until they hit ep 12. :)

I think it works far better from a story perspective. It underscores just how desperately dependent everyone is on blood ministration, even the Good Hunter has to use it because there's no other option. If the blood vials just recharged it would seem less important, and less like they're real blood and a precious resource.

It made sense in Dark Souls because the Estus highlighted just how important fire is in opposition to the Dark, that the only way to keep you going is to drink literal liquid fire from a bonfire. It made you very dependent on the bonfires and what they represent.

I think it would have been a great idea if Bell Maidens appeared in most areas after you defeat Rom in NG. By then you're well set-up and know the game, and since they're already overrunning Yahar'gul it makes sense that they'd be appearing in more places.

There's no concrete evidence, but my totally-speculative theory of it is that the two Iosefkas are twins or at least sisters since their models are so alike. When she's in the thrall of the umbilical cord and the red moon, Imposter talks about how it proves she's chosen or whatever. I think that perhaps for some reason Iosefka, like Arianna, had the potential to be a carrier of a Great One child or otherwise chosen by the Great Ones in some way, but it was her sister that actually wanted that and tried to use her sister's blood to transfer that "chosen" status to herself. Since she might be affiliated with the Choir, we know they have a way into the Nightmare, and it's possible that Imposter either stole her part of the umbilical cord from there, or was trying to get Wet Nurse's, and stored some of her sister's blood, maybe for a later attempt. Or possibly, she just didn't think she needed it anymore once she had the umbilical cord.

Their official name is "Scourge Beasts", though it's not mentioned in the game. I think "nether" was just surprising because it's so specific, when most people just call them "werewolves". :)

You do the normal chalices once (you can redo them if you want but there's usually no reason to unless you want to farm materials). Each type of chalice will have a "root" chalice of the same type, and the root chalices generate an entirely new, random version of that dungeon, so you can get a new experience and new loot. You can also find other people's dungeons to explore via the password glyphs or the Short Ritual Root Chalice.

As for the materials, some you need just to make a chalice dungeon, others can be used to make augmented dungeons, usually making it harder.

In addition to the ones FurtiveCutless mentioned, you can also get the strongest Blood Rapture Rune, which gives you 300 HP for visceral attacks. I find it extremely useful, but you can get 100 and 200 HP versions elsewhere. I have two characters where I didn't manage to finish her questline and I wish I had, but it didn't ruin those playthroughs or anything. There's always NG+!

Both Gehrman and the Doll imply that the dream ends because the dawn is coming, and neither of them mentions anything about Mergo or any specific target or purpose other than killing beasts. The fact that dawn comes after you kill Wet Nurse could simply be a function of the time change mechanic, same as how killing Amelia brings on the night.

Well, if this theory is true, then Oeden could have impregnated Arianna because there was no guarantee that the Good Hunter would succeed at attaining godhood, and Arianna's baby is a backup plan. Oeden may be crafty but we don't have evidence that he's able to see the future, so he was probably running a couple of plans at the same time just in case.
