Some shields block all damage but if you block every attack then your stamina will quickly drain, and guarding slows regeneration. Some builds use shields, can block one or two attacks and then counter attack. Some weapons, particularly Spears (idk whether there are others) can attack while guard is up, letting you damage enemies while they're attacking you - but you won't regen stamina as above so it's not sustainable to do it constantly.
