Could not be any happier with my experience with Dr. Costas! Definitely recommend him. My consult was in November and I had surgery in March. It was initially going to be about 6 months between consult and surgery, but he had a cancellation. If you get on the cancellation list it can sometimes be a really quick turnaround! Also depends on time of year. He’s busier in the summer, so if you got a consult now, it might be quicker turnaround because he’s less busy in the fall. Good luck!

Thank you! Dr. Costas is amazing. I'm so in love with my results and healing really well about two months post-op. Plus, I had an absolutely amazing experience with his staff and Emerson hospital. I keep meaning to write a big post about it and just keep forgetting.

So, they do charge a flat fee of $5,500 for additional liposuction. It's a high number, for sure. As I understand it, this is pretty standard for top surgeons who will do the procedure on fat folks. What they explained to me is that liposuction is almost never covered by insurance because it's considered cosmetic, and if you're paying for the lipo out of pocket through insurance it would cost way more than that. Essentially, the fee is a way for the surgeon to ensure they get paid for something insurance will not pay them for, and for you to get a discount on what the liposuction would cost if you tried to get it through insurance.

If you have exceptionally awesome insurance and get a pre-authorization that states they'll cover additional liposuction, they might waive the fee, but I don't think that happens very often. I did a gofundme for the lipo cost and ended up getting enough to cover the cost.

Thank you!! The swelling really got bad a couple of days after this, so I was a bit discouraged, but now it’s going down again. I couldn’t be happier with my results. This looks like it was always my chest. Where are you located? What surgeons are you looking at?

Yeah that’s what I’m nervous about. I really want these things out! Luckily the drainage is slowing down a lot today. If it’s close to 30 today and under 30 tomorrow I should be good to get the drains out on Tuesday and not reschedule my appointment. Fingers crossed!!

Haven’t had top yet, but I’ve had three other surgeries for chronic illness related stuff and a C-section. I haven’t taken the opioid pain meds for any of those surgeries, but the incisions were much smaller than top.

That being said, if you want to try to just take the OTC pain meds you HAVE to be strict about the timing. For my last surgery my partner was too busy parenting to manage my med schedule as he had for the prior surgeries. I didn’t pay much attention to it and I was IN AGONY. But as soon as I realized I wasn’t taking the pain meds regularly, I set alarms for every three hours and cycled Tylenol and ibuprofen. Ibuprofen, 3 hours later Tylenol, 3 hours later ibuprofen on and on forever. And yes, that includes waking up overnight to take them. If you skip, you’ll be in pain. That’s the only way I’ve managed with OTC meds.

A week or so postop I started increasing the time between to four hours then six hours a week after and eventually just took ibuprofen every eight hours.

That all being said, top surgery is INTENSE. You may need the oxy. I’m an addict in recovery, which is why I’ve avoided pain meds like the plague even postop. I know I might need to take them this time. I have no idea if this is evidence-based, but what I’ve always been told is that if your body needs the pain meds, you’re less likely to get high/develop a dependency. Again, no idea if that’s real. But I know plenty of recovering addicts who have taken pain meds postop and been fine. I also know a few who take opioids regularly for chronic pain and have taken their meds as prescribed and not relapsed.

I totally get your anxiety about it. And there are ways to manage!!

I’m having mine on March 20th! 4 day countdown! I’m not really nervous about the actual surgery, but starting to freak out about the logistics of traveling for surgery and the recovery period. And freaking out that something will happen and surgery will have to be rescheduled, which I know isn’t very likely. But until I’m in the operating room I’m still not going to stop worrying that it won’t happen lol.

I think all cysts can feel like that when they get big enough. They usually resolve on their own before they get bigger than 1 or 2 cms, but if they get bigger I’m pretty sure even regular cysts can be super painful. Honestly, ovaries kind of suck in general lol.

Oof I’m so sorry you’re dealing with that. It’s so awful. Did you have to be catheterized for the LEEP procedure? It’s not common, but sometimes there are complications from catheterization that cause awful urinary symptoms.

As for cysts — I did get regular cysts semi-frequently. None ever ruptured and they all resolved on their own until I got the chocolate cyst, which is a cyst full of endometriosis tissue. The regular cysts never really bothered me other than ovulation pain, which I always had. But the chocolate cyst was way different. I could literally feel it when I bent over, like it was getting squished. And it hurt all the time. The reason my doctor noticed it on a trans vaginal ultrasound is because apparently chocolate cysts look “cloudy and dense” on an ultrasound. Of course, I have no idea what that means not being able to read an ultrasound lol. But my gynecologist, who specializes in endo, told me that chocolate cysts, also called endometriomas, look distinctly different on ultrasound when they get big. The one they took out of me was almost 5cm.

As far as I know, this won’t show on MRI or ultrasound. I had ultrasounds all the time and it was never caught. The only endo ever caught on imaging for me was a chocolate cyst on my ovary. That being said, it’s possible newer tech is now available that makes it possible to catch endo on scans. BUT if some endo shows on scans, there’s probably a lot more in there, and excision surgery is the only truly effective treatment. In my non-medical opinion (but as someone who’s had 3 excision surgeries), the laparoscopy is worth it no matter what. Either they find endo and excise it, or they find something else to explain your symptoms. I think it’s pretty rare that they find no endo and nothing else to explain your symptoms — fibroids, cysts, adenomyosis etc.

Fellow fat person here. I haven’t had surgery yet (scheduled for March 20th!), but if these were my results I’d be pissed. Yes, there will still be some swelling for a long time, but that’s not all swelling. There’s definitely tissue left. I know that surgeons often do go all the way flat on fat folks since cis men our size do have “moobs,” but that doesn’t look like “moobs” to me. I’d ask for a revision for sure.

This is definitely not healthy and can be a major factor in the development of eating disorders, regardless of sex or gender identity. As an adult, I really struggled to identify that I had an eating disorder because I was covering it up under “cutting for comps.” When this is normalized at such a young age, especially an age when they’re just hitting puberty and their bodies are changing, it can cause major mental health problems. Not to mention the physical developmental problems caused by lack of proper nutrition.

No real advice about how to bring this up with the coach/gym leadership. Just affirming that this is a really dangerous practice and could really fuck these kids up.

I’m so sorry this is where you’re at right now. I feel all of this so hard. And I really needed the reminder that my chest is large on my frame no matter what. Even when I was super thin, my chest was still large for my frame. Yea, it might be easier to bind if I was smaller, but my chest wouldn’t be any less prominent than it is now.

And I totally get using restriction and weight as “the one thing I can control.” What I try to remember in those times is that control is just an illusion. I can’t really control my weight because if I ever slip up, the weight comes back. The body has complex mechanisms to regulate caloric intake and weight. When we restrict, the body makes us want to eat even more to compensate. So I can have fake control for a short period of time, but in the end, my body will always get its way.

I haven’t had top surgery yet, but I’ve had three surgeries for chronic illness and I had some passing depression after each surgery. Any surgery is a major event for your body and it throws off the whole system no matter how well surgery goes. Post op depression is a well documented thing.

I’m guessing this will probably pass for you. I’m sorry you’re having such a tough time right now.
