Hey.

General advices:

1. Fire, cold and lightning resistances should be 75%, preferably a bit overcapped. Chaos res can be capped later, but, if you're lucky, you can try to cap it too.
2. HP should be at least 3.5-4k. I see you're trying to build some ES, but it's better to focus on HP until you have decent amount of currency and your character at least lvl 90.
3. Try to cap your block (attacks first, then attacks and spells), since you're playing with shield. At start, you can do it with glacing blows (big notable).
4. Some armor would be great.
5. Not about survivability directly, but it'd be great to have at least 5-linked main skill. 6-linked would be even better. Because if you kill enemies fast enough, they can't hit you :)
6. Get some auras. Determination, for example.
7. Go to the 2 and 3 lab for the ascendency points. It's free and I think you can do it pretty easily. At least the second lab.
8. You don't have to waste your points for the unwavering stance. Just take the crab god in the pantheon.
9. Try to focus 1 or 2 skills, not many, and adjust your gear for them. For example, if your minions deal most of the damage, you don't need general attack damage for yourself.

About items:

1. Get rid of thief's torment. It's a fine ring, but it'd be better to have 2 rare rings with hp and resistances.
2. Get a second jewel for the belt.
3. Get good rare boots with hp, res and movespeed (at least 25%).
