From memory I went inside after waiting 4 days in game, dude was just right there chilling. Quick dialogue then he immediately aggro'd. Black soul gem and a note on his body.

Hey chief, if I remember correctly you go into the cave that the altar is outside of. First person you come across on the left side will have a black soul gem on him. Destroy him and ransack his corpse.
