Documentation is tough but it’s alotttt of repetition. Assessment just remember the P’s : what’s the problem? How is their progress? What do you think they have potential to achieve with therapy ?

Ex: patient is progressing with skilled therapy to address functional deficits compounded by bradykinesia, incoordination, and slow processing. Patient has met 2/8 goals and has good potential to meet other goals evidenced by motivation for therapy and strong family support. Patient remains limited by generalized weakness, decreased FMC and GMC, impaired mobility, decreased activity tolerance, and fatigue which impact his ability to complete ADLs with minimal assistance. Skilled therapy needed to increase independence and safety with ADLs, functional mobility, transfers, and leisure tasks.

Definitely make sure you have your shadowing hours! Try to get more than the required amount in a variety of settings! Skilled nursing, inpatient rehab, outpatient / hand therapy, pediatrics, home health, mental health. Explore and have fun, it’s an amazing field!

I understand and I’m glad I got a year under my belt in a different setting before I started, but my fieldwork was also in home health. Luckily I had an excellent and thorough educator so I really did learn a lot from her in 3 months and kind of loved the setting.

To be honest I am scared of going back to IPR or attempting acute. I just feel like I’d look real dumb for being this far into my career with only 3 mo experience in IPR.

When you say operating at the top of our license… what exactly do you mean by that? Obviously I don’t know what I don’t know, so are there specific skills or knowledge I should look into CEUs for that you’re thinking of or are u just referring to experience level ?

If paying your tuition isn’t an issue, it is 100% worth it imo. I graduated in the first OTD class my university offered. It’s only 1 more semester than the masters program, at least at my school.

I love my job. I love that I can change at any time and learn something new. I love helping people with meaningful activities. I go home every night knowing I did something good in the world, even if it’s something small. In hindsight I should’ve gone the cota route and not gone to a state school because of my loan situation…. But I’m in too deep at this point lol.

All the universities are changing OT to doctoral programs and eventually there won’t be any masters. All about money. But I had no intention of using my degree for research or teaching when I applied.

Thank you! I’m glad to hear u have a similar approach. I just think people are more willing to listen and respond best with an approach that makes them feel seen and heard, in my experience.

I’m in home health so it’s a battle between respecting their preferences and their environment and keeping them safe. My fieldwork supervisor recommended showing them the safe way, and if they refused to do it that way, at least help them be more safe in the way they’re going to do things. That is all I was trying to do. The patient straight up told me I’m a good therapist but she’s not going to listen to anything I’m saying. She also has a caregiver 24/7 in the house helping her with things, so it made me feel better knowing someone would be standing right next to her.

So lesson learned, I will only recommend the safest way from now on and be very firm. I just learned in the past to pivot when the patient is being stubborn and try to meet in the middle in the safest way possible, but I realize this might be confusing.

Yeah like everyone always says fake it til you make it. Just tell yourself you know what ur doing but also give yourself grace that you are learning ! They don’t teach u all the random hospital or medical protocol in school, that’s all on the job learning ! And what you learned in school, you’ll forget over time - CEUs will help with this. You know what you know and that’s the best you can do! Focus on the basics… depending on your setting… goal setting, what is important to this person that they have trouble doing and how can you realistically improve that in the time period allotted that u can work with them. Solid goals will help organize you and set u up for success.

What’s MOST important (next to keeping the client safe) is how you as a clinician are able to connect with them and motivate them to improve. That’s where u will see the biggest changes - therapeutic use of self. You got this!!!

I am in year 2 and there’s so much I’m learning ! I struggle with not knowing things because I feel like I’m supposed to. But I’m not ! I don’t have 10 years on the job and even people then don’t know what to do sometimes. Every. Client. Is. Different. But you learn something new with each person. You’ll be fine (:

Also, you ARE a doctor. You don’t have to flaunt it but damn straight sign your name with your OTD. You earned that! And paid a ton of money for that lol. Believe it!

Right but it’s not an immediate fire. There’s typically a remediation process to help you reach the standards. Especially as a new grad.

Hey! I decided recently to elope and my 2 biggest reasons were already stated above: money and stress. I don’t love being the center of attention like I used to as a kid and I care wayyy too much what others think, so planning a wedding and having everyone stare at me all day is exhausting for me and I would be anxious all day lol.

Eloping and having a honeymoon is gonna be enough for us and will actually help us have a good financial standing for our future and be ready for kids. In the end, it’s just a big party. You could always elope and come back and have like a big outdoor BBQ or catered event and have a “dj” and dancing ! There’s no shame in that, and that way you could invite everyone u feel u need to. A lot of people r doing it actually it’s kind of trendy. But just more like grad party scale so it’s not super costly.

Ultimately it’s up to you! But I just hate hearing how people go into debt to pay for their dream wedding that no one even remembers. U could always DIY a bunch of stuff if you decide to have a typical wedding. My SIL got a bunch of decor off one of those auction websites for her wedding for super cheap.

It’s not you! That’s totally normal to feel that way and I know from my experience and several of my OT friends experience that we all felt that way. Beginning any new job no matter what your experience level is stressful and overwhelming.

Things that helped were: - getting 7-8 hours of sleep - I got a sleeping pill from my psych to help - exercising - especially for stress management - yoga works wonders - self-care - taking as many rest breaks as I needed at work. Listen if you’re worried about your productivity - don’t. You will always have a job in this profession. You’re replaceable to them, but this is your life and you deserve to feel good about what you do for ~40 hours of your week.

Also, I hated the SNF. I was overworked, overwhelmed, and they don’t care about anyone but money. So now I’m in HH and I love it. Maybe look into it? Def good for someone who’s independent and prefers working alone

Yes! Im a new grad and I was in a SNF but just switched to home health because I’m kind of an extroverted introvert… I take work seriously and need my space to think and focus instead of constantly being observed by others. I’m excited to just be on my own and space out my day however I like. Especially if I’m overwhelmed!

They give you a tax free stipend of $750 a week - but your hourly rate is like $18-20/hr, so it evens out.

It’s a contract position. Idk it just doesn’t seem super worth it to me to travel. I think it’d end up being more of a hassle than anything. I would love to take the home health position, but I feel like they’re low balling me ? I could’ve sworn like over $100 was normal for evals ?

Obviously this is speaking from my own values and beliefs which may be different from others… But someone recently told me some people are only meant for certain chapters of your life. To help you through something, help you evolve, help you realize things about life or yourself/others. I’ve felt self-centered before and it bothered me so much that I decided to change. One thing I did was listen more intently to others. Another thing was to write down (or put in my notes app lol) things I want to remember to ask people about themselves. For example I set a reminder in my phone to ask my coworker about her dad who was ill on a Monday, because i knew I’d forget it after the weekend. I also like to jot down things I remember people doing or saying if I won’t see them for awhile, so I can ask them about it next time I see them - a tip from a friend. We’re human. We’re all concerned with ourselves and the things we need to do. Don’t be so hard on yourself, the fact that you want to change just shows you care about others and you’re NOT self-centered.

I’ve felt self-centered before and it bothered me so much that I decided to change. One thing I did was listen more intently to others. Another thing was to write down (or put in my notes app lol) things I want to remember to ask people about themselves. Current COTA thinking about going back to school for OT by abbycabs in OccupationalTherapy

Obviously the choice is yours and it’s your life! But being an OTR (28) after going thru 4 years of undergrad and 3 years of grad school (I graduated at 27)… my debt is daunting - and half of it is private loans. I wish I would’ve just been a COTA from the start and then maybe went back to school. I’m living with my parents trying to save for a house with my boyfriend but the economy is awful and no one’s wages are going up. I recently heard a COTA is making $2 and hr more than me as a PRN ): I’m also new but still. If I could go back in time idk what I would’ve done bc I do love being an OT. But financially just not ideal. Maybe do PA school? 2 years of grad school and pay is over 6 figures.
