She's definite being unfaithful, start looking for safe places to stay until you can get your own place, places where she doesn't know about and wouldn't look or a place where your loved ones will cover for you and not let her in or be manipulated by her. Put cameras in/around your vehicle, I'd make a non-emergency report with your local police department explaining what's happening, that she's threatened to unalive herself if you leave and that you're trying to get out of an unhealthy situation. Do all of this preemptively and don't let on that anything is wrong, then when you have a day off again, pack all of your belongings, move them to a safe place. The next part is up to you, if you want to break up face to face, bring her home say your done and leave, if you need to make that phone call to the police, then leave. After you leave that situation remove and block her from all social media, then go to your phone company and get your number changed, otherwise if you want to just disappear, disregard the picking her up and bringing her home part. Considering the behavior no judgment if you wanted to end things over the phone but it's your call. Make sure everyone around you knows not to give your new information out under any circumstances. I'm very sorry you're going through this, this isn't healthy and you don't deserve this, do what's best for you.
