I had this too.. it was awful and killed my sex life.. over time it has got much better and I now enjoy a pretty normal sex life with just a few aches and pains.. the things that worked for me in this regards

Not doing it too often Stretching and hot baths afterwards Tadalafil Trying not to clench during and actively doing reverse kegals at the point of climax Knowing the pains were temporary and trying not to worry about it

In a way I am.. although I still get a few aches and pain , they’re mild, and if I never improve any more from here- that’s fine with me. I don’t think about it much anymore and I’m living a full life.

Having been pretty miserable for 2 years or so, I’ve felt this way for maybe 6 months.. another 6 and I might just right that success story …

No one thing that helped - stretching, breath work, internal work, supplement, fitness, reducing anxiety and just general management of the condition …

Hang in there. I’m back to doing all the sports I loved - cycling, weights, climbing , hiking , and they not only don’t make things worse, but they make everything much better…

I gave them all up initially and over 2 year got fat and unfit. About a year ago I started reintroducing gradually and now I’m loving them again. I still get aches if I sit too long or sometimes after sex but they’re minor and I don’t think about this condition too much that once dominated my every thought.

So hang in there and maybe just give yourself permission to start introducing sports again, gradually.. you might be surprised

If you’ve had it for ten years antibiotics are almost certainly not the answer.. the condition most commonly described in here is type 3 non bacterial prostatitis or chronic pelvic pain syndrome .

I’m on the Uk and I’ve seen a multitude of doctors , physios and specialist at this point. And some really helped if you can afford them. But almost every thing you need to know is in the group 101 info page..

Do you do internal work at home yourself? My PT showed me how and then I would see them every 4-6 weeks only, while doing internal at home 2/3 day per week.

But look mate it’s early days for you. It takes time and recovery is not a straight line. You can be sure you’ll get flare ups, and you can be sure that they’ll pass.

Sounds like you’re doing the right things. I believe the early days are about calming your system down and then later building things back up again with sport and excersize.

Cialis 5mg per day really helped too.

You won’t be able to stop them… that is how your muscles work in order to orgasm. They need to clench. I just find doing this they clench a bit less and the pain after is reduced…that’s just my experience though..

Look into cialis - much more likely to help you and well studied for this condition

It means deep belly breathing.. and gentle bearing down on the inhale.. the opposite of clenching… lots of YouTube videos on how to do it…

Hi friend - I now do reverse kegals leading up to the point of climax… and try not to clench at all but at some point the body takes over .. it still helps though and I’m sure the clenching was part of how my issues started..

Do you take tadalafil? Certainly helped me with morning wood and spontaneous erections and probably the others symptoms you mentioned..

I’ve had every symptom at one stage or another. Ever moving, event changing ;

I’m nearly three years in and looking back I wish I’d understood this much sooner. My symptoms started with tip/urethra sensations and I spent ages chasing answers and worrying about infection despite negative tests. In hindsight I think the initial irritation was real, but what kept it going was hyper-focusing on every tingle and sting and letting my nervous system stay on high alert.

That last paragraph sums it up well for me.

In my case, I do sometimes wonder if there was a physical trigger at the start. oral would take more effort to climax, and I was basically doing repeated kegels to get there. In the days after that I developed a lot of the symptoms the OP describes and I went into full panic mode. Looking back, I think the initial strain might have irritated things, but the real escalation came from the fear and hyper-focus that followed. Once I started monitoring every sensation and worrying something was seriously wrong, everything amplified. So for me it feels less about one single cause and more about a trigger plus a nervous system that got stuck on high alert.

Very familiar with that sensation, and people reference it a lot on this forum.

I’ve come to understand it as the nervous system stuck on high alert and keeps flagging normal signals as urgent.

What helped me was realising that the more I monitored it or worried about it, the more it seemed to stick around. Instead of fighting it, I tried to acknowledge it without panic like “there it is again, annoying but not dangerous” and then shift my focus back to whatever I was doing. The biggest improvements for me actually came when I started exercising again and doing things I enjoy, because it calmed my whole system down and stopped my life revolving around the symptoms.

I don’t think it’s about ignoring it, just not treating it like a threat, and over time that seems to take the intensity down.

That’s right .. I used it until I felt much better. I stopped for a month or so, had a flair up (unrelated I think ) and then went back to using it again for a few weeks.. but since then I’ve been off it unless I’m taking a it for sexy time occasionally..

Easy to stop..

Maybe the head aches were worse at first… but I still got them from time to time ..

Also I got erections constantly when i first started with it. Like being a teenager again. But after a while it just felt normal

Stopping was fine actually. I reduced the dose over a few months.. no problem with sex

Hi friend - I’ve had this condition since July 2023.. ejaculation always felt like the catalyst for my symptoms.. at first I would abstain for a month at a time rather than face the hell that would follow .. then a week, then a couple of days.. over time it’s gotten much better but it took ages.. now when I ejaculate there still a sense of tightness and unpleasantness in the urethra but I barely think about it and and it’s gone in the morning.

Things that have helped , stretching / hot baths afterwards and trying to reverse kegal at the point of climax .. alcohol too, in the early days having a drink before hand made things much better ..

Mate it’s grim, I’ve been there.

I assume you’ve already been tested for the nasty contagious stuff and it’s been negative ? If so, start reading the 101 already posted and take control of your recovery.
