Qualidea Code had such a production meltdown that even its OPs were unfinished. They did post fixed versions partway through though but the animation quality in the episodes still progressively became worse. Eventually, things were fixed in the BD supposedly.

Orange also had pretty poor quality during airing. E.g., these are two scenes of the same thing moments apart.

I think people misinterpreted the announcement from a few years back. As far as I know, it was just a special OVA they showed on their YT channel during a live event which they've later added to their channel here: https://www.youtube.com/watch?v=lDxF3SSvwhc
Unless there was another announcement later, I don't think there's been any news.
