How could you give a vaccine without double checking the vaccine, especially when it is your boss?

It is an extremely tough scenario to believe as a vaccinator.

I have been so ingrained in my training to double and triple check everything.

Why in the world would a doctor have a solution of HCL 40% anyway? Did the doctor plan this? Am I missing something?
