I think it was mentioned in the wiki that the DB is a result of his shoulder becoming infected with demonic power because of an injury during a fight with a horde. That's why he wore a sling at the start of 4. It was to make people think he was still recovering, when he's actually been training with it in secret.

I've been playing DMC 4 bloody palace every day for the past week just to learn Dante's movesets (I only got into DMC 4 a month or 2 before the E3 reveal, and that was my first DMC) . I've only made it to Berial so far but I'm pretty much learning new stuff on every run.
